Agreed. The xrays do not look normal after b/c we can't give them what God gave them. Need to have a very serious and realistic conversation that this is just buying time and DJD is coming. There is literature on young hemophiliac ankle fusions, they do we'll considering. Anyone aware of studies showing outcomes with <18 y/o ankle fusion (not hemophiliac)? Please share. This is a rare, but serious problem and I think any reasonable attempt to salvage the joint is warranted (with realistic expectations) OATS, De Novo, Prochondrix, nano-fracture etc, all on the table. What is NOT on the table here for me is "En Bloc" talar allograft. Anyone else feel the same? Different? Good discussion to have. Good Case.
Massive Talus OCD/AVN This is the case of a 16 year-old who sustained a #TalarNeckFracture and Medial Malleolus fracture as a result of a dirt bike accident. He was treated with ORIF of the talar neck fracture and medial malleolus fracture, which resulted in #AVN of the talar body laterally. I was consulted to treat the resultant AVN. ❓ How would you treat this large area of AVN in a 16 year-old ❓ He was treated with open debridement of the talus, drilling of the talus defect, proximal tibia autograft, and #Cartiform Allograft with #BioCartilage filling any gaps. His most recent XR are 7 months out from the above. He is back to running and cutting, some soreness but only with extensive activities. Is it perfect on XR? For example, we know that likely (and qualified) total joint patients are: over the age of 50, have active lifestyles (including golf, tennis, yoga, pickleball, etc.) and about 12 other demographic and psychographic data points... We link the persona to a geography, such as this 30 mile radius around Knockville and *POOF* we have our audience: these 420,263 individuals. We serve ads directly to these people for your services, in the places where they already go online… Without them ever performing a search. WHY? ➡️ Because the best way to influence consumer bahavior is to be in front of your most-likely audience, on repeat, without them performing a search. In March of this year, this exact strategy generated 228 new patients for one orthopedic practice. Isolated MCL tears are generally treated without surgery. This case is a bit unusual but worth sharing. This patient is young and suffered a medial tibial plateau fracture which was fixed at an outside hospital. He unfortunately developed further MCL laxity after this which was confirmed on stress views and on arthroscopy. He complained of pain from the plate and continued laxity. We did a plate removal and a MCL reconstruction with an Arthrex fiber tag tightrope for tensioning and added an internal brace. Check out this new surgical technique below.
