June Image of the Month: Choroidal Osteoma Choroidal osteomas are classified as choristomas, normal tissue in an abnormal location. They are typically present in the posterior pole, commonly peripapillary, as a yellow-white to orange choroidal lesion representing a tumor in which mature bone replaces the normal choroid. If the tumor is not macular, the patient is often asymptomatic. Diagnosis is often confirmed by the presence of calcific ossification on B-scan ultrasound or CT scan. Most are observed without treatment. Lesions can be associated with the development of CNV, which can be treated if they arise. Chest radiograph in a 2-year-old child shows a focal contour bulge (arrows) along the left upper cardiac border due to protrusion of the left atrial appendage. Chest radiographic differential diagnosis includes partial pericardial defect, cardiac tumor, and left juxtaposition of the atrial appendages. MRI shows marked dilatation of the left atrial appendage. There is a sharp indentation (arrow) at the junction of the left atrium and appendage (LAA). Deterministic Effects!! * The biological effects, the severity of which depends on the dose received are termed as non -stochastic or deterministic effects. * The deterministic effects exhibit threshold doses, so they can be avoided by limiting the exposure to the levels lower than the threshold doses. * It will appear after some threshold dose, below that it will not come. * Severity increases with dose due to cell killing (severity proportional to dose) * Only can happen in radiation accidents and radiotherapy patients (Acute) * Examples : cataract, infertility, erythema, epilation etc #radiation #effects #deterministic #severity #dose #threshold

Chorioangioma of placenta 3D -Hypo- or hyperechoic, well-circumscribed mass -usually located underneath the chorionic plate near the umbilical cord insertion - protrudes into the amniotic cavity. -Color Doppler demonstrates large vascular channels around and within the tumor, feeding vessel can be usually seen -may undergo infarction/resolution with advancing gestation -increase in size may lead to fetal complications(anemia etc) so monitoring is important #drnupurshah #fetalmedicine #chorioangioma

A 9-year-old patient suffered a pathological fracture in the neck of the right femur, which should be treated with internal fixation as a relative emergency, with a sample of the tumor being taken for histopathology. The tumor/cyst is cleaned and sent for histological examination. The process of reducing the fracture was performed with initial stabilization and curetting the site of the tumor/cyst in preparation for filling it with bone substitute material, as well as with autograft bone taken from the epsilateral fibula bone. This was followed by fixation of the fracture with internal fixation with a plate and locking screws.
