#The_Boston_Declaration_2025 #Global_Neurosurgery has emerged as a distinct academic subspecialty within neurosurgery. Although, Global neurosurgery activities have been in place for decades, there have been no central organizing efforts to improve the harmonization of these endeavors until recently. The 2016 Bogotà Declaration on Global Neurosurgery was the first of such initiatives. However, recent evidence demonstrates that wide gaps remain in access to safe, timely, and affordable neurosurgical care. Quite as important is the current dominance of global neurosurgery discourse by High-Income Countries actors. The Boston Declaration seeks to further define a unified vision of progress as global neurosurgery continues to grow and evolve. This ambitious initiative will review existing evidence, employ on-the-ground expert experience, and seek broad inclusivity and transparency to formulate a new set of goals for global neurosurgery and a structure that shifts the agency to Low-and-Middle-Income Countries actors. We propose a path to developing a new consensus statement and action plan, the 2025 Boston Declaration for Global Neurosurgery.

In this recently published article in the Journal of Neurosurgery Publishing Group, Zachary Olmsted, MD, PhD (Resident, PGY-2), Peter Wu, MD (Resident, PGY-4), Michael Dorsi, MD (Assistant Professor of Neurosurgery and Director of Community Neurosurgery), and colleagues explore the impact of poorly positioned leads in the intrathecal space during spinal cord stimulation (SCS). SCS has emerged as a commonplace treatment for patients with chronic low back pain. While lead migration is a well-elucidated complication of this procedure, the impact of malpositioned leads is less defined. In this study, the authors investigate two cases where surgical revision was necessary following incorrect placement of leads during SCS. Published in Neurosurgery, the Mount Sinai Cerebrovascular Center studied anesthesia modalities used for endovascular thrombectomy (EVT) in distal and medium vessel occlusions, comparing conscious sedation (CS) with general anesthesia (GA) for distal vessel thrombectomy. The research aimed to uncover which method provides better outcomes for patients suffering from acute ischemic stroke. Led by Drs. Christopher Kellner, Reade DeLeacy, Johanna Fifi, J Mocco, and Shahram Majidi, Amol Mehta, and Daryl Goldman, and researcher Preethi Reddi, key findings include: • CS was associated with higher odds of early neurological improvement. • CS led to higher rates of successful reperfusion complared to GA • CS showed a trend toward lower mortality rates (although not statistically significant) • Both CS and GA were found to be safe and feasible with no significant differences in 90-day outcomes, intracerebral or subarachnoid hemorrhages, and first-pass effect Conclusion: CS during EVT for distal and medium vessel occlusions is safe and may lead to better immediate outcomes, offering potential benefits for early neurological outcomes and successful recanalization. 🧠✨ Check out the full study in Neurosurgery: https://bit.ly/4bujxAl
