Our speaker for this session is Sophie Lester from ILS Case Management who has over 10 years of experience as a Case Manager working with adults and young people following a complex, life changing injury. She has worked with these clients and their families setting up, developing and managing directly recruited care teams. World Hospice and Palliative Care Day, held every year on the second Saturday of October, is an opportunity for communities around the globe to unite and recognize the importance of compassionate end-of-life care. ◆Palliative care is specialized medical care for people living with a serious illness. This type of care is focused on providing relief from the symptoms and stress of the illness. It is now officially proven that families heal and stay healed when clients attend Legacy. How to read the graph- The numbers range from 2 to -2. With 2 being high clinical distress and all values below 0 meaning that the family system has returned to the healthy range. Families entering Legacy tend to have a value around 1 (clinical distress) and finish Legacy around -1 (well into the healthy range). In addition, families stay in the healthy range for at least a year after completing treatment.

In the field of healthcare, we often find ourselves in family conferences where we must deliver news that no family wants to hear—that, for their loved one, the only path forward is through palliative care, as there is no current medical solution. This disclosure goes beyond scientific knowledge; it is both an art and a profound skill that every clinician should strive to master. To sit with a family, to bring compassion to painful realities, and to offer comfort when hope for cure has faded—this is one of the most challenging, yet human, aspects of our calling. Our role is not only to treat illness but also to honor the lives and emotions of the patients and families we serve, guiding them through unimaginable moments with empathy and integrity. Palliative care may not always offer cures, but it offers dignity, comfort, and connection.
