You HOPE they’re not “one of those doctors” who is aggressive with procedures or tests just to make more money. But you don’t really know. Word of mouth helps, but it only goes so far. If we’re being honest, everyone in healthcare is rewarded for “doing more”. As a spine surgeon, I get paid more when I do more surgeries. Pain docs get paid more to do more injections and interventions. The spine industry (Medtronic, Stryker, Globus Medical, NuVasive, DePuy Synthes, etc) gets paid more when patients get more implants. And hospitals get paid more when more beds are filled and more “billable events” happen. It’s called “fee-for-service” for a reason. I’m definitely not saying that specialists should get paid less. Quite the opposite. We should get paid MORE. When the Centers for Medicare & Medicaid Services continuously ratchets down physician reimbursement year after year (and the American Hospital Association blocks physician-owned hospitals)… it just forces us to do even more “units of care” to make up the difference. The healthcare system is a game where maximizing incomes has almost nothing to do with maximizing outcomes. Master surgeons don’t get paid more for a surgery than below-average ones. Conservative surgeons get paid less then aggressive ones. We surgeons get paid for how much we do, not how much good we do. Is this sustainable?
