🔥An avoidable death where the issue of unsafe #prescribing has been ignored🔥 This feels like a conspiracy of silence. As a #healthcare professional, who taught on the independent prescribing course, I find this shocking. This was not appropriate for a #LeDeR (learning from deaths) review. A full independent review should have been triggered. I believe if the National Prescribing Centre had not been abolished following the disasterous 2012 NHS reforms, this would have been picked up. There are also similar cases of deaths where the prescribing should have been scrutinised.
