A recent publication in Science Translational Medicine highlights a new tool aimed at improving the understanding of cerebrovascular issues that lead to strokes.    Researchers have developed a miniature probe that fits within the microcatheters used in brain surgery, providing high-resolution images from inside cerebral arteries. The study involved 32 patients with diverse cerebrovascular conditions, including aneurysms, ischemic strokes, and arterial stenoses.    By providing a clear view from within the vessels, this technology could substantially impact the diagnosis and treatment of cerebrovascular diseases.   #healthtech #innovation #healthcareinnovation

3D Whole-Heart Imaging in Cardiovascular #MRI: Exploring the Range of Clinical Applications by Eva Nyktari, MD (Cardiovascular MRI Unit, ONASSIS CARDIAC SURGERY CENTER, Athens, Greece); et al.   Initially introduced for coronary artery visualization, 3D whole-heart navigator-based acquisitions have been used especially to delineate complex congenital anatomy and morphology. The technique is superior to other imaging modalities used in congenital heart disease (#CHD), as it is free from ionizing radiation and does not require breath-holding. It also allows the acquisition of full-volume data including the entire heart and thoracic vasculature at a specific point of the cardiac cycle (usually end-diastole) with high SNR and isotropic voxel resolution that enables reconstruction in any plane during post processing. Apart from its value in anatomy and morphology, free-breathing high-isotropic-resolution 3D whole-heart LGE imaging has been established as a valid method of tissue characterization in a wide range of ischemic and non-ischemic cardiomyopathies. Recent major developments in #electrophysiology (#EP) #ablation procedures for #arrhythmia treatment have raised the need for novel 3D whole-heart high-resolution LGE techniques to image thin-wall cardiac chambers such as the atria and right ventricle. Technological progress has made it possible to both identify the arhythmogenic substrate and illustrate its underlying architecture. This information has been used to aid or even guide EP procedures, increasing both safety and success rates. The authors focus on using 3D whole-heart imaging in adult congenital heart disease and arrhythmias, and on how it helps the invasive cardiologists on the Structural Heart Disease team and on the EP team in their daily practice. Let me share an interesting study that I just read, highlighting the marginal advantages of Machine Learning over traditional models in predicting cardiac surgery mortality ℹ️ Zeng J, et al. . Eur Heart J Qual Care Clin Outcomes. 2023 : https://lnkd.in/ehWyxaY5 📈 Key Findings: - Analysis of 168,565 adult cardiac surgery cases - Both temporal (2013-2018) and spatial validation methods  - Overall in-hospital mortality rate stood at 1.9%. - In temporal testing, the top ML model showed an AUC of 0.797 vs 0.791 for the LR model's AUC  - Variation in feature selection methods had minimal impact on the performance of ML models, and a significant number of both ML and LR models were found to be miscalibrated. 💬 Discussion: - Only a marginal improvement by ML models over traditional approaches in predicting mortality preoperatively with routine clinical variables.  - Suggests that the predictive performance of ML models may be limited when working with basic clinical information due to the low dimensionality and lack of complex interactions among routine clinical variables 🔜 Advantages & Future Directions: - The focus should shift towards improving model calibration and updating existing models for better clinical applicability, considering the miscalibration and limited discrimination difference found in most models. - To surpass this limitation, it may be necessary to implement changes at the data level rather than the model level

🌐 Exciting Research Update! I’m thrilled to share my recent article titled: "Intraoperative comparison between strain elastography and pre-operative MRI features in high-grade gliomas using fusion imaging: a pilot study" published in World Neurosurgery. https://lnkd.in/d2BBjc7b The study compares elastographic patterns of high-grade gliomas and adjacent healthy brain tissue on intraoperative #ultrasound, with pre-operative #MRI characteristics. The study found that #glioblastomas typically show peripheral enhancement and necrotic patterns on MRI, with heterogeneous elastographic patterns, while anaplastic astrocytomas exhibite solid, non-enhancing MRI patterns and homogeneous, stiff elastographic patterns. Clinical Neurophysiology 2024;163:102–111. https://lnkd.in/g7zBQCwn Editor's Commentary: While stereo-electroencephalography (SEEG) is commonly used to localize the seizure onset zone in patients being evaluated for epilepsy surgery, it also offers unique opportunities to investigate brain functions through focal stimulations and recordings that are not possible with non-invasive methods. In this volume of Clinical Neurophysiology, Ayra et al. reported the results of multimodal investigations that include focal stimulation using SEEG electrodes, recording during auditory tasks and analysis of resting state connectivity. Changes in facial expressions and perioral movements were elicited with stimulation of the transverse temporal gyrus and adjacent cortex (TTG+) in about one-third of the patients studied. Did you know that today is the birthday of Dr. Charles Dotter; "The Father of Interventional Radiology"? Born on June 14th, 1920, Dr. Charles Dotter first proposed the idea of interventional radiology in 1963. The following year he opened a new era of percutaneous angioplasty through accidental operation, marking the formation of interventional radiology. On this basis, the techniques of balloon catheter dilation and metal stent implantation were developed. The endovascular stent was proposed in 1969. In 1973, percutaneous angioplasty was a breakthrough with the emergence of the soft double-lumen balloon catheter. Internal medicine, cardiology, cardiothoracic surgery, radiology, gastroenterology, nephrology, neurology, neurosurgery, and gynecologic surgery all rely on his interventional techniques to treat patients. The tenets presented in Dotter's 1964 article 28 which describes the 1st interventional case still hold today. The photo below shows Dr. Dotter hand-injecting contrast medium into his patients in the radiology suite.
