Camden Coalition has a bit of a cult-like status within complex care models, and the work there is ground-breaking, mission-oriented and thoughtful. Given my admiration for them, the studies showing their lack of demonstrable results point to a simple fact: Care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this [really, any] complex population of high-need patients. Primary care has proven its value. I am more concerned that a lot of the innovative, Specialty-focused models seem to rely on navigation, coordination and peer supports - without learning the lessons that are tried and true about complex populations. The reality is that these populations actually need a place to go when their needs escalate, a primary care provider to help them and real specialist access/integration for their needs. Let's hope that the next wave of these innovative companies learn these old lessons - these populations deserve it. https://lnkd.in/ecsjcbY2
