It is exactly 277 years since the British Navy surgeon James Lind wanted to treat scurvy, a lethal disease at the time, and tested different potential cures on 12 sailors. ⛵ He allocated the men into six groups and treated them with different medications. Lind evaluated the results of different groups, following the principle of comparing like to like: the treatment results were the best with the group that was given oranges and lemons. 🍋 Lind's groundbreaking work is considered the first clinical trial, which is why 20 May is celebrated as Clinical Trials Day. The National Center for Rehabilitative Auditory Research (NCRAR) was established in 1997 at the Veterans Affairs (VA) Portland Health Care System (VAPORHCS) in Portland, Oregon. The NCRAR is one of 13 research centers supported by the VA Rehabilitation Research & Development (RR&D) Service. Each of the RR&D centers focuses on a specific health condition that is of relevance to United States military Veterans. The NCRAR is the only RR&D center that focuses on auditory and vestibular research. Whereas other researchers are focusing on finding a cure for tinnitus (permanent elimination or reduction of the tinnitus sensation), the NCRAR’s focus has been on developing and testing different methods of tinnitus clinical management with the overall purpose of validating clinical methodologies for routine application. We thank the Ministerie van Defensie and the internal innovation team for the invitation to showcase KAIROS in the session of “Innovation for the healthy and the injured”. In military settings, where rapid response and immediate decision-making are paramount, KAIROS can enable accurate and personalized antibiotic treatment strategies. The integration of KAIROS into military medical practice has the potential to save lives, optimize resource utilization, and strengthen the resilience of military healthcare systems. Together with our partners we will be exploring this further. When did the first randomized clinical trial occur? Today is #clinicaltrialday, with a history dating back to the 1700s, precisely 1747. James Lind, a naval surgeon in the Royal Navy of the UK, conducted one of the first clinical trials on scurvy. He tested 12 affected men, providing citrus fruits to some and none to others. Those who received citrus fruits reported the most immediate impacts. Last week, we held an investigator meeting for our clinical study Maestro, focusing on Inflammatory Bowel Disease (IBD). This discussion involved experts like Marla Hochfeld, Jim Cucinotta, RRT, MPH, Eric A. Hughes, MD, PhD, and Walter Reinisch, exploring the complexities of IBD and the role of clinical trials in the treatment paradigm. What?!? My diagnostic test has to work outside at -40 degrees in the dark?!? The MHSRS is a fabulous forum focused on learning the Warfighter's very unique medical needs. I've been developing medical products for decades and none of them have had product requirements like these!! Yet they could have if we just knew about them. Thinking of the DoD as a potential customer, and designing in these requirements early in a product's development, are a great way to make more robust products that impact more lives and increase sales. This is a shocking and eye-opening statistic that we need to talk more about and make a change. The fact that in 2022, out of 55,188 applicants to AMCAS®, only 375 were veterans and 164 were active-duty military, representing a mere 0.007% and 0.003% respectively, highlights a critical gap in our approach to integrating the invaluable skills and perspectives of our service members into the healthcare sector. The vision of SOF to SOM, to harness the excellence ingrained in the Special Operations Forces (SOF) culture is noble and necessary. Our initiative doesn't just aim to bridge the underrepresentation gap; it seeks to enrich the medical field with the unique leadership, resilience, and problem-solving skills that SOF veterans possess. These qualities are especially critical in addressing future healthcare challenges and bringing valuable perspectives to both military and civilian medicine. Adaptive Sports Medicine: A Clinical Guide 2nd Edition I would like to thank the many contributors and authors who have added to the advancement on adaptive sports medicine. In addition to updates on the previous chapters there are new chapters specifically on Pediatric Adaptive Sports, Concussion in Para-Athletes, Military Veterans in Adaptive Sports, and others.
