The scale of healthcare organizations is out of control. Hospitals should be locally run, not subsidiaries of megasystems. Physician practices should be run by doctors who live in the same community as their patients, not managed by PE companies (or Optum). Having spent over 30 years in the healthcare industry, I’ve observed that certain patterns tend to repeat themselves. Many attempts to bridge the gap between providers and insurers have been made, but most have ultimately failed. Providers often seek closer ties to premium dollars, while insurance carriers focus on controlling care delivery for cost reasons, leading them to buy clinics and hire doctors. This isn’t a situation where you can be “partially pregnant.” During my time at hospitals, we began each day by reviewing the previous night’s census—essentially, the number of patients in beds. Similarly, at insurance companies, we tracked daily metrics like bed days per thousand to assess hospital occupancy. Understanding these dynamics is crucial for driving meaningful change in healthcare. You can either control costs or profit from patient care, but I haven’t seen anyone effectively do both.

Construction, maintenance and mechanic services have some of this problem as well but healthcare takes the crown. I'd go a step further than this article to say so much of the money that is cited as being spent on "#healthcare" is actually being spent on administration, financial services, inefficiency, overhead and profits (and not always the profits rewarding the innovators, researchers and clinicians).

California’s health care system is in crisis. Hospitals are closing. Emergency rooms are overcrowded. More than 40 hospitals have stopped offering labor and delivery services. Patients wait months to see a doctor or specialist. Care for 15 million children, seniors, disabled and low-income families on Medi-Cal is significantly underfunded. And the state has repeatedly redirected more than $20 billion in health care funding to non-health care purposes. Prop 35 will protect and enhance our health care system for all. Prop 35 will secure ongoing revenue by extending an existing tax on health insurance companies — without raising taxes on individuals — and dedicating these funds to protect and expand access to care for all Californians. And it will prevent the state from redirecting these revenues for non-health care purposes. https://lnkd.in/g8NpuayU
