In honor of the #NASS Conference next week I put together a quick snapshot of our #Top10 #SpineSurgery procedures and their utilization in Illinois, along with a few thoughts. The report is my own (notes in the comments), but it is based on commercial & Medicare claims data and does not encapsulate the full market size (though I’d venture to say it’s more than directional.. message me directly with any questions). A few observations which may complement your own experiences: *** 1.6 claims-to-patient ratio suggest patients can often have multiple spine surgery claims in one year; not a surprise to seasoned surgeons who know that complex interventions can lead to complex care pathways/rehabilitation and follow-up procedures ***A lot of different specialties are involved in and around spinal surgery claims. Ortho-trained surgeons lead the charge on performing the procedures (and in this data set the top performers averaged around 300 spine surgery claims per year) ***BUCA’s are the dominant financial class for patient, and I was surprised to see the average-charge-per-patient was not crazily higher than Medicare prices (data not shown, but stay tuned) ***Unsurprisingly the 65-84 age group is the largest patient cohort (and growing with our aging population = market will get bigger in the near term) ***The transition from a predominantly hospital-based surgery to the ASC setting is official Want to know a few more of the key drivers behind the data? Initially, the DCS has been described in severe liver trauma associated with coagulopathy. Over time, Due to the observed advantages, the DSC approach has become standard practice for abdominal trauma with the extent to extra-abdominal trauma and acute abdominal emergencies. Currently, despite the widespread use by the surgical communities, the indications of DCS strategies have not been clearly defined in the absence of high data level and concerns have hence risen about the overuse and related adverse outcomes of DCS approach in surgical trauma patients. However, Advanced progress in trauma-resuscitation techniques have significantly improved the outcomes and reduced the requirement of DSC strategies in trauma injured patients. In sum, DCS remains an important strategy to surgically manage a specific cohort of patients. Surgery is an “indivisible, indispensable part of health care,” yet the global need for equitable access is growing, especially in low-to-middle income countries (LMICs). Despite the fact that one-third of global disease is surgically treatable, global surgery and anesthesia remain critically underfunded and overlooked in the global health sector. Limited access to surgical and anesthetic care is a leading cause of disparities everywhere, including the United States, where vulnerable groups experience poorer surgical outcomes. Did you know that wrong-site surgery is still happening? In 2022, there were 1441 sentinel events reported, and it's disconcerting to note that wrong-site surgery accounted for 6% of these incidents. It occurs in various medical fields, with orthopedic cases taking the lead at 35.3%, followed by neurosurgery at 22.1%, and urology at 8.8%. Even in the age of “Time-Out” procedures, these statistics underscore the need for heightened awareness and preventive measures in these specialties. The real costs of this error go beyond just the dollar amounts. The most common human costs include the need for additional surgery (45.6%), pain (33.8%), mobility dysfunction (10.3%), aggravated/worsened injury (8.8%), death (7.4%), total loss (7.4%), and scarring (7.4%). The significant harm to the patient is the real cost. “Across all surgeries, the overwhelming top contributing factor to [wrong-site surgery] was failure to follow policy/protocol…[but] only 14.7% of claims were related to a need for policy/ protocol,” the study authors wrote. “This suggests the main issue lies not in creating policies but in the implementation of a policy/protocol, including the use of the World Health Organization’s Surgical Safety Checklist.
