Santa Clara Family Health Plan extend our sincere appreciation to the many providers and their dedicated teams who attended our annual Provider Meet and Greet, hosted by our Provider Network Operations Department on October 16, 2024. Providers were warmly welcomed by Chief Executive Officer Christine Tomcala and Chief Medical Officer Dr. Ria Paul, M.D., who engaged with attendees throughout the afternoon. The event brought together 85 representatives from various provider organizations, fostering a valuable opportunity for networking and collaboration. Representatives had the opportunity to speak with each other, and learn about the impact everyone’s work has on SCFHP members. Some of our contracted vendors attended such as QUALITY HEALTH PARTNERS and Protoqual, providing valuable information on their services. Finally, there was an added bonus to the afternoon - attendees enjoyed complimentary lunch, branded swag, and raffle goodies! “We relish a wonderfully collaborative relationship with SCFHP,” said Ivan Gonzalez, Assistant Manager at Yellow Checker Cab Company, Inc. of Silicon Valley, a transportation provider for SCFHP. “It has been integral to our shared success. Over the years, regulatory and compliance requirements have changed. And, we’ve worked hand in hand to ensure the transportation model has evolved to comply with those changes. SCFHP’s willingness to consult with us on process-building is a testament to their trust in us as a provider and the strong partnership we’ve cultivated over many years of work together.” Santa Clara Family Health Plan thanks all our providers and office staff for their unwavering dedication to delivering high-quality services to our members. If you’re a provider, learn more about our provider network at https://lnkd.in/g7UNDdk7.

Imagine you're at the doctor's office, feeling under the weather and overwhelmed. You wait for ages, finally get called in, and then... it feels like you're just another name on a list. We've all been there. But what if there was a way to transform this experience from frustrating to empowering? John (let's call him that), a retired carpenter with a mischievous twinkle in his eye, was no exception. Frustrated with a string of appointments and tests, John felt like his healthcare journey was a maze with no clear path. Then came Sarah, a patient care coordinator with a knack for making connections. Instead of just rattling off medical terms, Sarah took the time to understand John's concerns. She explained things in a way that was clear and easy to follow, even for someone unfamiliar with medical jargon. More importantly, she listened. John, initially hesitant, found himself opening up about his worries and daily routines. This conversation was a turning point. No longer feeling like a nameless patient, John became an active participant in his healthcare. He asked questions, understood his treatment plan, and even felt a sense of control over his health. The next time John went for tests, he wasn't just going through the motions - he was informed, engaged, and even a little optimistic. The Secret Ingredient? Patient Engagement. Patient engagement isn't just fancy talk for appointments and reminders. It's about building trust, clear communication, and empowering patients to take charge of their health. Think of it like working together towards a common goal – a healthy you! Studies show engaged patients have better outcomes, from managing chronic conditions to sticking to treatment plans. They're also happier with their overall healthcare experience. How can you, as a patient, be more engaged? Ask questions! Don't hesitate to clarify anything you don't understand. Take notes or bring a companion who can help you remember key information. Be open and honest about your health concerns, habits, and lifestyle. Don't be afraid to discuss your goals and preferences with your healthcare provider. By working together, patients and healthcare professionals can create an effective and satisfying healthcare experience for everyone. Just like Sarah helped John navigate his health journey, patient engagement allows us to move from a system focused on numbers to one centered on people and their unique needs. Remember, open communication and a little effort from both sides can lead to a healthier you! #PatientEngagement #PatientCareCoordinator #Healthcare #HealthcareAdvocacy #PatientExperience #Communication #Empowerment #Wellbeing I'm Anulika Ufoh, a passionate Patient Care Coordinator dedicated to creating exceptional healthcare journeys for patients. GPs: Today is World Family Doctor Day. A quick reminder to give yourself the same care you give your patients! #WorldFamilyDoctorDay Far from being selfish, self-care is a priority if GPs are to maintain a healthy work-life balance, avoid burnout and do their best on the job. For GPs, prioritising self-care looks like: ✔ Enjoying healthy snacks and eating mindfully ❌ Grabbing food on the go and gobbling at your desk ️✔ Making time for family ❌ Putting work above all ️✔ Scheduling refreshment breaks ❌ Cramming in consultations ️✔Digitising to streamline space and time (we can help!) ❌Working in a cluttered general practice. ❗Reminder: You can use your Strengthening GP Grant to digitise your patient records. The deadline for general practices to spend their grant funds is 30 June 2024. #UseItOrLoseIt #generalpractitioner #medicalrecords ##documentmanagement #documentscanning #gpgrant

I am thrilled to announce that we have founded WorldClassHealth.com: Global Healthcare for Global Employers! We're on a mission to enable global employers to provide their employees access to the world’s best healthcare providers on a global marketplace. The core tenets of our approach: (1) high quality of care, (2) a concierge patient experience, and (3) reduced costs to both employers and employees. I’m honored to be part of an incredibly talented founding team, which includes Siddharth Nambiar (Director Amazon, Co-Founder Rocket Internet), Dave Guilmette (CEO Aon, Cigna, WTW) and Serkan Kutan (CTO ZocDoc, Amwell, Amazon). Our advisory team reflects our commitment to our mission: Our Clinical Advisory Board includes Mohan Chellappa (President Emeritus, Johns Hopkins International), Sharon Kleefield (Harvard Medical School, AIIMS, Healthcare Standards Institute), and Erick Kazarian (Rothman, NYU Langone, Mayo). Our Go-to-Market Advisory Board consists of Brian Marcotte (BGH), Steve Putterman (Mercer), Jeff Bernhard (Aetna), and Paul Matthews (Towers Watson). Our Technical Advisory Board consists of Naresh Shanker (Xerox, HP), Lalith Vadlamannati (Hinge, Amazon) and Babak Parviz (Amazon, Google). On the legal, regulatory and ethical front, we are advised by Dale Van Demark (McDermott), Tamim Bazzi (Cooley & Co), Ryan Shaening Pokrasso (SPZ) and I. Glenn Cohen (Harvard Law School). We are actively hiring across Product, Tech, Clinical Quality and Ops, and Go-to-Market: https://lnkd.in/eSE4CcET. We are addressing a fundamental problem in healthcare: clinical quality and pricing are not transparent, and access is limited due to high co-pays and deductibles. There is no simple way to engage the highest quality and best value global healthcare providers. There is a clear opportunity for a concierge patient experience with Global Centers of Excellence. With the WorldClassHealth.com platform, employees can choose from healthcare providers across top global destinations that meet the highest quality bar while costing significantly less than local options. Self-insured employers can drive down their overall healthcare spend and share savings with employees by waiving co-pays completely. We believe the time is right for WorldClassHealth.com to make a significant impact on how care is delivered globally. I am fortunate to have the opportunity to go on this journey to help employers and patients with a great group of co-founders. More to come soon!

In healthcare we treat events that have undesired outcomes as special events, that need to be dissected and understood. I propose the events are a direct output from the work. “A system will produce what it is designed to produce” paraphrased Berwick …. Like a speeding ticket we all break the limits some more than others on occasion that work produces a speeding ticket. ( undesired outcome)The question is was that a special cause or just one time in a system that happen to produce a bad outcome? Let’s work in the work systems maybe even before the events…to promote true safety!
