Important study with implications for monitoring renal function

Closed Loop Medicine's dose-personalizing smartphone app increases hypertension treatment success in study with Pfizer drug: 📲Closed Loop Medicine aimed to show that combining prescription drugs with dose-optimizing software could help personalize the treatment of high blood pressure (increase BP control, reduce side effects & increase adherence to medication) 📲According to the company, about one in five patients do not refill their first prescription of the drug, largely due to dose-dependent side effects such as swelling of the legs and ankles. 📲Closed Loop’s smartphone app aims to help dial in the correct amount of drug a per-person basis, enough to control blood pressure but minimize side effects, based on patient reported data 📲The study, published in the JAHA — Journal of the American Heart Association, examined Closed Loop’s app paired with Pfizer’s first-line blood pressure med Norvasc (amlodipine). 📲The 14-week trial of about 200 participants showed that average home blood pressure readings fell from 142/87 mmHg to 131/81 mmHg - with about one-third of patients demonstrating control with the study’s lowest amlodipine dose, titrated to 1 mg per day 📲About 88% were able to control their hypertension with no ankle / leg swelling while adherence rates in blood pressure recording and medication were 84% and 94%, respectively 📲Closed Loop described the results as a stepping stone to other therapy areas / drugs, such as GLP-1s for diabetes and weight loss. It’s ironic that the number of opioid prescriptions in our small practice has increased markedly over the past years in the midst of an ongoing “opioid epidemic.” It’s no secret that this epidemic has been going on for decades due to inappropriate prescribing, pharmaceutical pressures, and suboptimal patient selection. The increase we are seeing in our practice is predominantly in the older patient population given Medicare’s cripplingly limited policies for interventional care (injections/non-surgical spinal procedures). Managing patients on chronic opioids is high risk and cost at miserably low reimbursement rates with increased exposure for potential audits by multiple agencies. It’s striking, confusing, and infuriating how many referrals we receive for “opioid management” from local hospitals – you know them well.  Upon receipt of such referrals, we will call the referring party and are told “we don’t have providers that manage chronic opioids here.” During this ongoing opioid crisis, it is my humble opinion that this is not acceptable. Given the tax breaks/facility fees/advertising budgets/legal resources that hospitals enjoy – these terms do not apply to a small private practice – it’s egregious that hospitals don’t roll up their sleeves like the rest of us helping to manage patient safety on a daily basis when it comes to opioid management. We can all save more lives by working together. Hospitals: use your abundant resources to hire well trained, compassionate physicians that can manage your patients that are maintained on opioids. Patients seek you out for excellent, “comprehensive” care as you so proudly advertise - provide it.

Cohort study of the effects of inpatient creatinine testing frequency on acute kidney injury identification and staging. Results highlight the need for changes in the established hospital procedures to ensure more frequent testing of serum creatinine levels.
