GP Pharmacist | Consultant Pharmacist | Talks about #multidisciplinarycare #ruralhealth #pharmacy #generalpractice *Credentialed Pharmacist of the Year 2024*
The second annual National Symposium for Pharmacists in General Practice is FAST approaching (THIS Sat 10th Feb, 2024) Now, I’m not embarrassed to admit that some Googling was necessary, and my “research” tells me that a "#symposium" is a specialized meeting where #experts discuss and present #research on a specific topic, often fostering in-depth dialogue. It differs from a conference by its focused theme and emphasis on #collaboration rather than diverse topics. So join myself, one of the abovementioned experts, along with a slew of other GP Pharmacists and others in the industry talk about GP Pharmacist things. If you're a pharmacist that has been thinking about entering the multidisciplinary team space, this event is for you. If you are a GP Pharmacist and you want to connect with others in this rapidly expanding role, this event is for you. If you are a pharmacy student that thinks that it's Community, Hospital, Industry or bust, this event is for you. If you are a #doctor, #medicalstudent or #practicenurse and you want to know what having a GP Pharmacist in your practice is all about, this event is for you. If you are someone who wants to impress someone else and say, "yeah I'm attending a Symposium this weekend", then this event could potentially even be for you?! Register FREE at the link below. My session has been pre-recorded because absolutely no one trusted me to stick to my time limit (reasonable), but I will be online live during the event for question time and to respond to questions in the chat. Can't wait to see new faces and of course the familiar ones who've been flying the GP Pharmacist flag for some time now. "Everywhere medicines are, pharmacists MUST be". https://lnkd.in/gpBJb6gT Pharmaceutical Society of Australia The Royal Australian College of General Practitioners (RACGP) Australian Primary Health Care Nurses Association (APNA) Australian College of Nursing (ACN) WentWest Murray PHN #GPPharmacy #GPPharmacist #mdt #multidisciplinaryteam #teambasedcare #generalpractice #pharmacy #napsa #pharmacystudents

Australia's first cohort of candidate Aged Care Onsite Pharmacist (ACOP) and Medication Management Review (MMR) pharmacists have commenced the Pharmacy Care Academy's credentialing. I extend my gratitude to Regis Aged Care Sandgate for hosting our work-integrated-learning. Coal-face exposure is the only way to effectively prepare pharmacists for the practicalities and complexities of working within the residential care setting. #industry_preparedness #career_opportunities #medication_safety_champions

Happy world pharmacist Day I would like to thank all of the pharmacists for their dedication and hard work. They play a vital role in our healthcare system, and we are grateful for their service. Here are some of the important work that pharmacists do: 1.Pharmacists play a key role in ensuring that patients receive the right medications at the right dose and at the right time. They review patients' medical records and prescriptions to identify any potential interactions or side effects. 2.Pharmacists provide counselling to patients on how to proper use the medications. This includes explaining how often to take the medication when to take it, and any special instructions for taking the medication. 3.Pharmacists monitor patients for any adverse effects from their medications. If a patient experiences an adverse effect, the pharmacist can work with doctors to adjust the medication or prescribe a different medication.

Elevate your health and quality of life with personalized medication management and caregiver support. Let's optimize wellness together! | 𝗦𝗲𝗲 𝗙𝗲𝗮𝘁𝘂𝗿𝗲𝗱 𝗦𝗲𝗰𝘁𝗶𝗼𝗻 ⬇️
Pharmacy 💊Tip # 6: For seamless Care Transitions 🚑. Navigating hospital admissions can be complex. As a patient or caregiver of a patient, you should be engaging with a pharmacy technician and/or pharmacist at several points to streamline care and keep medication management on point: 1. Upon Hospital Admission: Ensure a pharmacist or technician captures a detailed medication list, including over-the-counter drugs and supplements. 2. Transferring Within the Hospital: A pharmacist should verify that your current medications align with any new orders as you move between units. 3. Before Hospital Discharge: Pharmacists play a crucial role in educating patients and caregivers about the home medication regimen, setting the stage for successful recovery. 4. First Post-Admission Doctor Visit: Within 48 hours post-discharge, expect a follow-up call from a healthcare professional to review medication adherence, address queries, and remind you of upcoming appointments with your primary care provider. This proactive approach ensures continuity of care, keeps you informed, and significantly reduces the risk of readmission. 💡 Remember, your pharmacist is a key ally in your healthcare journey. Stay engaged at every step! #healthcareconsulting #healthbenefits #transition #caregivers #caregiversupport

These 2 reasons are why new PCN pharmacists should focus on building their confidence first. Have you started or planning to start a new role as a clinical pharmacist? Confidence played a big role in my first few months in role. Here are two reasons why i think you should focus on building your confidence as soon as you start. Reason 1: Having confidence in your ability can help overcome imposter syndrome Reflecting on your days and focusing on the positives helps with this. Instead of berating yourself for your mistakes. Focus on the positives. By reflecting on the good in the day, you'll be able to identify where your strengths lie. Eventually helping you get over imposter syndrome. Reason 2: Having the confidence to say no can help prevent stress and worry It's important to learn to say no when you don't feel ready to take on more advanced work. Have the confidence to say to your lead pharmacist "I'm not ready for that yet but can you help me work towards that". While It's important to push yourself into your stretch zone to help growth, be careful that you aren't stretching your limits too far too quickly. By asking for advice on how to work towards becoming competent in more advanced work, you break the process down into smaller, achievable steps. Through these processes you'll be able to avoid stress and worry over your ability. Allowing you to take on any challenge that comes your way.
