Post-Mortem Analysis is necessary for both failures, successes, and the more common "in between" scenarios. All are "teaching moments", key is to ensure that your failures are not fatal ones (hard to learn from 'dead' state, unless the game has auto-rez feature or your quest party includes a cleric/healer of suitable caliber to revive the recently deceased.)
