CONSUMERS, BEWARE OF FRAUDSTERS AND MERCENARIES in the hospice space: Program integrity is so important amidst the fraudulent activity in the hospice space. But there are ways to separate the “wheat from the chaff” so to speak. Hospice consumers deserve to know which providers are trustworthy and are in hospice care to help people die comfortably and with dignity. Hospice companies who are in the business for the right reasons do not need to worry about the need to escape “the ever-growing fraudulent activity in the space.” As my father always said: The cream always rises to the top.” Regulators know which companies are in it for the wrong reasons but if they do over-regulate to compensate for that—they will harm the good companies. There are providers who start a hospice company to capitalize on the volume of people entering their last years of life; only to flip the company in three years, pocket the profit, and move on. And then, there are others who plan to invest in hospices as a long-term money-making enterprise until they reach their personal financial goals. That is one of the reasons why I wrote my book called, “Surviving Hospice: A Chaplain’s Journey . . . It explains how to discern which providers place patients and their families as their primary stakeholders and which companies use dying people as a vehicle to generate a lot of money. I provide the critical information on making a good hospice selection in Part II of my book. No other hospice book includes this information. If you enjoyed the book, “The Big Short,” written by Michael Lewis, my book is narrated in the same format: Using patient stories to highlight the mercenary nature of investor-owned hospices. It’s unfortunate that the bad apples have given hospice care a black eye. I was determined not to let that be the final word. Hospice care is the best way to have a good death—if, you have a reliable partner who wants to uplift humanity instead of feeding the greedy monster within. No medical care is perfect; including end-of-life care, but I list the top nonprofit and for-profit hospices in the State of Wisconsin. The guidelines I give for choosing a good provider will work in every state. Do not be afraid of the idea of hospice; but do be wary of the fraudsters and mercenaries who have hijacked this amazing benefit. Unfortunately, the hospice company that hired me in 2010, was in it from the start with a very specific agenda: grow their company, sell to a private equity firm, recapitalize, and rake in the money. The original owners had expensive taste and opulent dreams. The vision was always about placating a hunger for experiences, real estate, and new business opportunities. I don’t usually care what folks do with their hard-earned money; but, not when it’s at the expense of the dying, their families, and the loyal hospice clinical teams who pour their hearts into compensating for those who maximize profits by reducing services to pts. www.hospicehelppro.com

The points here are that unbiased guidance begins with people who do not have a competing financial or business interest and that community resources with knowledge of local service providers are a huge asset.
Those online senior living referral services are getting a lot of attention these days, and not in a good way. The chair of the U.S. Senate Aging Committee has commenced an investigation into assisted living referrals by A Place for Mom, one of the biggest commercial advice outfits.

This is why I do what I do. By the way, Centene's profit was $2.7 billion in 2023. How many more people will have to die so that companies like Centene can make obscene profits? It's the law for all privatized government plans and in most states as well: if the network is inadequate, they HAVE to pay in-network level benefits to an out of network therapist. Don't depend on employees of companies like Centene to know this. They receive minimal training (and salary to match). Find a therapist and demand the benefits you pay for. If you need help, there are advocates out there who will help!

Here are some shocking facts regarding the senior-care referral business within California: Anyone can go into business as a senior-care referrer, no qualifications required. Senior-care referrers have absolutely no obligation to disclose their financial relationships with the assisted living communities to which they refer their clients. Senior-care referrers are not required to perform any due diligence on the communities they recommend. They don’t even have to confirm that these communities are licensed! I take due diligence very seriously As a Certified Senior Advisor (CSA®), I follow a very detailed process to ensure the communities at which I place my clients meet my high quality standards. Here is my process: Step 1: Check Licensure I always start at the California Department of Social Services’ licensing website, to review all recent visits that their inspectors have made to a community. If there have been minor infractions, I speak with the facility’s owner regarding how these were resolved. If there have been severe infractions, I immediately take that facility out of consideration. Step 2: Visit Site Whether it’s a six-bed residential care home or a large assisted-living facility, prior to contracting with them I visit the community and interview them based on a five-page Care Capabilities Checklist. I’m not just looking around to check for cleanliness and atmosphere. I need to know what level of care they are trained, willing and staffed to provide. While there, I tour the entire community, take photos and ask to speak with residents (and their family if they happen to be visiting at the time). Step 3: Continuous Reassessment Once I am considering a community for placement of a particular client, I revisit the Department of Social Services’ website to obtain updated information. After all, things can change, and with that there can be problems at a facility that I previously would have considered a viable option. Step 4: Communicate with Community Prior to touring a family, I communicate with the senior community owner or manager regarding the specific care needs of my client. Are they capable right now of managing these care needs? Step 5: Bring Family on Community Tour You can bet that while I am there, I pay close attention to what I see. Step 6: Follow Up Once I place a resident in a community I return within the first week, and periodically thereafter, to speak with them and their family about their experiences. This feedback then gets added to my database for referencing going forward. Not all advisors in my field take the same approach As this recent expose in the Washington Post explains, it can be pretty scary out there! Yes, the reason I am able to provide my services at no cost to my clients is that, as I fully disclose, I receive a commission from those communities in good standing at which I place my clients.
