Whether he's playing basketball, football, soccer, golf, baseball, or just imagining epic adventures, Steel always sees the glass half full and spreads joy to everyone around him! 🏀🏈⚽️🏌️♂️⚾️✨ Born with bilateral clubfeet, Steel faced many challenges and years of treatment at various healthcare facilities. Despite these efforts, he still struggled to run without falling. Seeking a second opinion, his family turned to Scottish Rite for Children. Upon learning Steel's clubfoot had relapsed, they moved from Kentucky to Dallas for two months, allowing Steel to receive weekly castings and surgery, paving the way for his remarkable recovery. From the moment they walked in the door, every doctor, nurse, therapist, volunteer and cafeteria worker at Scottish Rite made Steel and his family feel welcome, understood and special. Dr. Riccio went above and beyond to address every question and concern they had. Balancing Crawls! Crawling is a great way to get some weight bearing though the shoulders, arms, and hands. But do you ever notice your clients moving so fast the activity is not activating the muscles you were going for? Try this balancing activity to slow them down! Place objects around the room for your client to pick up or simply place the objects on your client’s back and ask them to crawl without letting the objects fall. This encourages them to use slower, controlled movements. Make it imaginative by pretending the client is a train, horse, or on an amusement park ride! Use this as an in clinic activity in combination with a play theme or obstacle course or suggest this for an at home activity! Interesting that this continues to be a question but glad there are folks setting the record straight in OKC in ACL-R. Particularly with the quad tendon graft and the challenges we are seeing related to that graft type. How long until current practice and general provider understanding reflects the literature??
Got an email today from another PT colleague - "Dan, settle a debate in ourc clinic...can you do long arc knee extensions on acute ACL reconstructions or not?" YES...and you should! Depends on a few factors to be clear, but here, I have a high school soccer player who is 2 weeks post-op with a quad tendon graft. She is actually supine (didn't show it completely to protect her identity) - this is one of the nuances of QT grafts - need less trunk flexion to get the rectus. Lots of recent literature supports doing long arc knee extensions, contradicting many of the studies in the late 90's. No resistance today, but will progress to that as well as using NMES w/ BFR. I'll be talking about the various pearls with different ACL grafts as part of my All Things Lower Extremity course coming up on October 19th in Overland Park, KS. Approved for 8 CEU's in KS!
