I've come a long way after making a 10-fold medication error in 2005 (something I kept to myself for a long time). Now, almost 20 years later, and with 15 years of helping others avoid the same mistakes, I recognize what it really takes to treat the critical pediatric patient. It won't happen with a spreadsheet. It won't happen by downloading an app on your phone. It won't even happen by taking a PALS class every 2 years. It takes a commitment by the organization, from top to bottom, and from the bottom up. If the EMS Chief tries to force it, it won't work If the medic doesn't believe in it, it won't work. If you are just checking boxes, it definitely won't work. Here's what it takes: 1. Leadership that recognizes how hard it is to dose medications, both for kids and adults. 2. The commitment to address the issue. 3. A dosing system that is 100% customized to the agency protocols. 4. EMS specific education that focuses not just on protocol knowledge, but also on how to speak to families, assess children, and administer medications. 5. Continuous quality improvement to evaluate the process on an ongoing basis. The recently released National Prehospital Pediatric Readiness Project (PPRP) does a great job helping EMS organizations evaluate their level of readiness. See the video below for more information on this. It takes a village to ensure that our patients are safe, and we need to all join hands and help each other out, because.... When a child comes back to life, it's all worth it. When you prevent an overdose (like I made) it's all worth it. When your crews feel pride after running a difficult pediatric call, it's all worth it. We are ALL IN on helping those who want to see this change happen.
