The information gathered will support clinicians to help people see the right clinician, first time. This is being piloted in a handful of trusts to evaluate the impact and scalability of this technology across NHS secondary care. Hospitals across the U.S. face capacity issues, leading to unnecessary stays and higher costs. A February 2024 analysis by Kaufman Hall shows that reducing length of stay boosts financial performance. Their April 2024 report noted a 5% drop in average length of stay since 2021, though it's remained flat year over year. 🏥 Kaufman Hall emphasizes that hospitals with effective structures and technologies for improving length of stay gain a financial edge. Inefficient discharge processes, staff shortages, and lack of post-acute beds extend patient stays. A 425-bed hospital could save over $20 million annually by cutting unnecessary inpatient days, while also creating space for new patients, potentially adding $20 million in margin. "Decreased continuity of care during hospitalization had been associated with poor outcomes or prolonged length of stay. Weekend usual provider continuity (UPC), which represented the fraction of the weekend days with a clinical note by a primary inpatient attending physician, has been used in internal medicine service. Higher weekend continuity of care was associated with reduced length of stay. Goodwin et al. proposed using a weighted mean of schedule continuity for hospitalists. A higher quartile of schedule continuity was associated with lower 30-day readmission rates, lower 30-day post-discharge costs, and higher rates of discharge to the home." #Readmissions is always a focus of #hospitalists and this study provides some additional insight. The study is limited in evaluating a single center, malignancies and a limited definition of holiday but there is value in considering the impact of scheduling and patient outcomes. #hospitalmedicine #acutecare #cancercare

While there are dissenting voices regarding the AMA's choice to consolidate inpatient and observation codes, many physicians find it logical. From a physician's viewpoint, distinguishing between inpatient and observation status isn't a clinical determination. The clinical decision revolves around whether hospital admission is necessary, a distinct query altogether. For most physicians, the tasks and clinical reasoning remain unchanged regardless of whether the patient is admitted as inpatient or observation status. If the differentiation between inpatient and observation vanished, my responsibilities and clinical considerations would largely remain consistent, albeit with fewer peer-to-peer discussions and conversations with utilization management. #UShealthcare #MedicalBilling #Physicians
