📣 To kick off National Health Center Week, August 4-10, the White House issued a proclamation recognizing the key role Community Health Centers play in making sure health care is a right in this country, not a privilege: "Federally funded community health centers are a lifeline for over 30 million Americans nationwide — providing affordable access to quality medical, dental, and behavioral health services, which save lives. Community health center sites are especially critical for people in Tribal, rural, and underserved communities because they are often the closest and most convenient sources of care. During National Health Center Week, we recognize the importance of the nearly 1,400 federally funded community health centers that provide critical services to Americans. We express our gratitude to their dedicated staff, who tirelessly devote themselves to the well-being of their patients. Our latest research paper, "The Role of Street Medicine and Mobile Clinics for Persons Experiencing Homelessness: A Scoping Review," has been featured as a cover story in the International Journal of Environmental Research and Public Health! Homelessness is a critical issue, resulting in an estimated 5,800 to 46,500 deaths annually. The connection between experiencing homelessness and poor health is undeniable. In our scoping review, Mobile Health Map researchers explore how mobile programs and street medicine—where care is delivered directly to persons experiencing homelessness (PEH)—can mitigate this crisis. By providing trusted, affordable, and accessible healthcare, these innovative approaches show immense promise in improving health outcomes and reducing healthcare costs. Public housing agencies, including those in Boston, Seattle, and Austin, are addressing resident health through #innovative partnerships, reflecting the impact that housing has on physical and mental #health. For example, Boston’s Healthy Start in Housing program, launched in 2011, helps low-income women with high-risk pregnancies by providing stable housing and health services through collaboration with the Boston Public Health Commission. Seattle’s Behavioral Health Outreach Program, established in 2019, supports public housing residents with behavioral health issues through personalized care from nonprofit service providers. Austin’s housing authority partnered with federally qualified health centers to open a comprehensive healthcare clinic. These efforts suggest that, through resident engagement, health services utilization data, and partnership building, housing agencies can play an important role in improving health outcomes for residents. Great meeting this morning with the American Heart Association. There are so many great opportunities in Kansas City to support and advocate for marginalized communities. I propose the following as immediate measures that healthcare and government institutions can do to help the marginalized (right now): • All health equity strategies MUST be trauma-informed and involve cultural competency as evidenced (for example) by the inclusion of “unintended consequences” forecasting during strategic planning. • Hospital systems and Federally Qualified Health Centers MUST grow closer together. Gaps in care within Kansas City’s failing patient referral system are costing us precious time and lives. A local, primary care coalition involving every institution in the chain of care is critical to success. We can do this. We all need to do this. Trust me on that one. • Case Management remains severely underfunded across the board. Whether through Community Health Workers, Cultural Health Navigators, or Social Workers, the Kansas City Community needs to continue developing a wrap-around, assisted patient experience for residents of high mortality zip codes. “You were born where? You live where? At Axis Community Health, we're committed to ensuring equitable access to quality healthcare for all. That's why we were excited to visit the Sacramento Capitol today, advocating for policies directly impacting our patients and clinic. During our visit, Axis’s Chief Medical Officer, Dr. Dawnell Moody, addressed the critical need to tackle the workforce crisis in primary care in the United States emphasizing the importance of preparing the next generation of primary care providers to serve diverse patient populations. Dr. Moody further discussed how Community Health Centers play a vital role in providing essential and cost-efficient healthcare to millions across the US. She outlined challenges faced by Community Health Centers in California, particularly regarding financial sustainability. Our behavioral health team provided insight into the multi-faceted benefits of integrated behavioral health highlighting our Axis Bridge urgent mental health intervention. Dr. Jennifer Penney, Axis’s Chief of Integration, shared the program’s effectiveness in providing urgent mental health services to individuals of all ages as well as the significant cost savings achieved through Axis Bridge, as well as its role in decreasing hospitalizations and unnecessary emergency department visits within the Tri-Valley. Oral health equity is a pillar of health equity, and among our policy priorities. This new report on oral health needs throughout New York State, released with several of our partners, highlights high-need areas, especially in rural counties. "The research team identified 31 high-need areas with significant oral health needs, primarily located in rural regions such as the Mohawk Valley, North Country and Southern Tier, where more than two-thirds of the population live in high-need areas. Similarly, over one-third of the population of the Central New York region resides in high-need areas." https://lnkd.in/eTq54sMQ

National Health Center Week (NHCW) is an annual celebration that highlights the significant role of health centers in providing comprehensive and culturally sensitive healthcare services to millions of Americans. For 2024, NHCW will take place from August 4 to August 10. Dr Minesh Patel, GP and NAPC Non-Executive Director attended the Frimley Health and Care Integrated Care System ‘Inspiring Primary Care Transformation’ event last week to lead two sessions on ‘Population Health and Communities – Integrated Neighbourhood Teams’. Minesh said: “We had a very engaging discussion on population health improvement and making neighbourhood care real, looking at the keys to success including working upstream and the importance of activating teams and activating residents. "We discussed examples of simple upstream interventions around common underlying drivers of health demand such as diabesity and effective and enabling wellbeing interventions such as singing groups, supporting people living with frailty and the impact and ended on the benefits of the group consultations model as a way of engaging and activating people with some examples from my practice. “Frimley is a dynamic ICS with some local challenges, but they realise the importance of improving working lives and are taking positive steps to doing this. They have well established population data and analysis that they are actively using which is really positive. It was great to see Professor Claire Fuller there talking about neighbourhood care and Dr Phil Wallek, one of the Primary Care Home leaders from Lewes also talking about the importance on real team working and benefits of demand and capacity planning around keeping well.” There are some examples of case studies focusing on population health improvement on the NAPC website: https://lnkd.in/daqyuCSP
