Stop and Watch Early Warning Tool Stop and Watch is an acronym which arranged vertically provides a list of condition changes that staff can observe for and document by circling the letter and communicating the possible change to nursing staff. When an acute change in condition occurs, the Stop and Watch Tool can be used by CNAs to identify changes in residents and clearly communicate those changes to the licensed nursing staff. https://zurl.co/srUe
