(All opinions are mine and mine alone) I’ve been part of many successful Epic EHR implementations over the past 20 years, often led by IT but closely partnered with Operations. This collaboration has always been vital, but it prompts an intriguing question: Should EHRs continue to be seen primarily as an IT domain? Yes, we’ve made strides with informatics teams that bridge IT and Operations, and many EHR committees are operationally governed. But here’s a thought – is partnership and shared governance enough? Or is there a case for shifting EHR ownership more decisively towards Operations? Consider this: while IT plays a critical role in maintaining EHR systems, the real impact of these workflows is felt in Operations. In IT, our approach to resolving workflow issues is often systematic, akin to hardware fixes, involving tiered helpdesks and multiple approval layers. Contrast this with the fluid, responsive nature of operational workflows in healthcare settings. As a nurse, I’ve seen how vital operational autonomy is – from adapting care plans to escalating patient needs. What if EHRs were approached more like an Operations Department? Envision a system as integrated and responsive as the relationship between pharmacy and nursing or respiratory techs and patient move teams. This isn’t about diminishing the value of IT; it’s about reimagining ownership to enhance responsiveness and efficiency. I’m not claiming to have all the answers, but I believe it’s a conversation worth having. How can we move beyond partnership models to a more operationally owned EHR system? While reading an article about the Epic vs Particle Health issue, one paragraph caught my attention. “Epic, a 45-year-old privately held company based in Wisconsin, is the largest EHR vendor by hospital market share in the U.S., with 36% of the market, according to a May report from KLAS Research. Oracle is second at 25%, following the software company's $28 billion purchase of Cerner in 2022.” Epic represents a 36% market share and Oracle Health (Cerner) a 25% market share. That’s a 61% combined share in the EHR space. And I have a feeling that gap will close up some with several of Oracle’s bigger contracts like the VA and IHS contracts among other potential clients. I am curious to know what the remaining 39% consists of. But the point of my post is something else. Connectivity. I understand that there is a large price difference between Epic and Cerner/Oracle Health, and not every healthcare organization may be able to afford one, or either of these platforms. However my biggest beef with opposing systems is the lack of communication/patient record portability. Epic has the ability to connect multiple MyChart accounts and share patient records between organizations seamlessly. For me this has been helpful as I moved from Cleveland, where I worked at and used Cleveland Clinic as my healthcare organization. I moved to northern Indiana and started going to Parkview for care and I was able to with the tap on my screen, seamlessly move my records to another organization. Cerner’s Healthelife portal lacks any kind of similar capabilities, which to a technophile like me is disappointing.. but what’s even more frustrating is the lack of connectivity between organizations on Epic and Cerner. A typical patient wouldn’t know any different and wouldn’t think to look at what EHR is used.. they just find a new facility when they move.. so they may not know the process required to get medical records transferred between providers. Epic/MyChart make it super easy… Oracle Health/Cerner do not, and going from Epic to Cerner or the opposite is just as frustrating. In a world of connectivity I feel that’s a critical step in helping patient care be more accessible and portable for those who may move or travel and so on.
