Compliance when no one is looking ... Most people have been on a plane and have seen people ignore the safety briefing. After all plane travelling is about the safest method of transportation known. In case you are interested, travelling by mule is about the most dangerous. But that’s a different story. But when a crash happens, it often impacts many passengers and crew. If people do their own thing, rather than follow the well-designed emergency procedure, the results can be catastrophic. I was struck by the recent plane crash in Japan when JAL156 hit a coastguard plane. Unfortunately, 5 of the 6 on the coastguard plane died but all 300 on board the JAL flight survived. Why? I think you can see this as a compliance success - a mixture of skills and drills kicking in and an entrenched safety culture. Much of the culture has been put down as a response to a crash in 1985 when JAL 123 crashed with only 4 survivors from 524 passengers. An unenviable record of the deadliest single aircraft accident. The root cause was put down to a faulty repair of a tail strike in 1978 and since the crash, JAL has put safety at the centre of their operation. For example, all new joiners spend time in the JAL museum to understand why the JAL 123 crash occurred and how the work of everyone in the firm can reduce/eliminate future risks. On the day, staff knew what to do and passengers did what they were told. They reacted to the circumstances but relied on their skills and drills. In moments of stress, people descend to their training - there is little time for reflective evaluation. Within 7 minutes, all the passengers were off the plane. No one carried off their luggage. No one was run down by a fire engine. Everyone on JAL 156 survived despite the plane rapidly becoming a flaming inferno. In regulated firms, there are lots of tasks that need to be done by a whole range of staff orchestrated by a compliance team. Just like the issues the cabin crew have with passengers, some of the front-line staff may not understand the importance of some of the compliance work. Or even, be ‘nose blind’ to what is needed for the job in front of them. It’s difficult to organise frequent ‘skills and drills’ by teams in ‘high-risk’ environments, but there is a lot to be inspired by. We know we need to train staff in our specific requirements. Generic ‘your firm’ training is just not fit for purpose today. But are you being sufficiently curious to ensure that staff are really engaging and not being physically there but mentally absent? How can you build skills and drills? Are you using checklists? Great - then are you enabling time for reflection? Do you have a robust quality assurance process where checks are done close to the point of production? Can you involve the front line in some of these checks to reinforce ‘the right way’? It's a disaster waiting to happen, and indeed it almost does. A sudden burst in a pressurized line leads to the trailer body's catastrophic descent. By sheer luck, or perhaps a guardian angel, the worker scrambles to safety, narrowly escaping what could have been a fatal accident. This incident isn't just a close call; it's an alarm bell about the perils of suspended loads. All too often, individuals in various industries find themselves working under or near loads suspended by cranes, lifts, or even something as seemingly harmless as a raised vehicle bed. The potential for disaster in such scenarios cannot be overstated. The questions we must ask ourselves are poignant: Are we taking unnecessary risks? Have we become complacent about the dangers of suspended loads? It's a matter of 'what if', not 'if'. Safety protocols exist for a reason. They are the culmination of years of experience and too often, lessons learned from tragedies. Using appropriate and rated equipment, adhering to safety guidelines, and constantly being aware of our surroundings - these are not just recommendations; they are necessities. Following factors should never be overlooked but implemented in every organisation: General training and development of staff , quality checks ,Job analytics assessment ( Technical , non Technical, maintenance , customer service , safety , security,first aid , supervisory , advisory in short employee development and competency renewals with on job training before they can handle the duties and responsibilities solo . After all their dedication and hard work contributes significantly to the smooth operation of train Services . unfortunately Malpractices exist and some network operators fail to conduct proper background check of their employees which leads to mismanagement and issues where in most projects end up being failures . No defined hierarchy. No proper channel of communication or command . No advisory support . Politics , Nepotism , Hypocrisy , Micro Management Improper or poor general management , Wasting of resources , Less salaries for technical safety critical job positions ,Nepotism & favoritism,No work life balance , Delayed trainings or job skill development . Delayed responses in almost everything ,lack of proper railway clause and procedures at workplace , poor work culture & environment,Lesscareeropportunities ,improper work schedules , underappreciation,overbearing department leadership , incompetent managers , lack of proper communication , Highly toxic work environment , no recognition for accomplishments ,slavery & dominance mindset of managers & non accountability of HR department as there involvement in human resources development must be the modus operandi . The System Safety Challenge As we complete the first quarter century of this millennium, many of us considering ourselves “system safety experts” find ourselves in a conundrum in responding to the many recent high-profile safety issues. Design issues, quality problems, human errors, new technologies, AI and other challenges elicit no shortage of strong words and opinions on what the problem is and what to do to move forward both from inside and outside the system safety profession. The variation in “what to do” from the “safety experts” can confound any well-intentioned person trying to get a warm feeling that the industry is headed in the right direction with regards to safety. The problem is exacerbated by the fact that so many of these various safety experts can be fully or partially right, even if it sounds like they’re saying vastly different things. As a system safety certification instructor at the University of Southern California Aviation Safety and Security Program, I have a chart I like to show students that illustrates just how wide the definition of “safety expert” is. I ask students to circle where they are on this chart and in what capacity they operate and connect the circles with a line. Then I say that each top to bottom line can represent a different interpretation of what system safety is and result in safety experts sounding like they’re disagreeing when in reality, they are not. While I’m sure this chart can be expanded to some degree, the point is that depending on where the product you are developing fits into this chart, your “safety expert” may have different applications of safety standards, safety processes, and even terminology.  While there are underlying fundamental truths about system safety, each “line” you draw from top to bottom for your specific application can carry its own unique path of how to make/keep a safe product. The cross-hairs of the system safety challenge lie with those in the “Role” line of the above chart. They are the risk managers, decision makers and recipients of the consequences from an unsafe product making it to the marketplace. The problem is that the definition of what a “safety expert” is for each of these roles really doesn’t have a firm industry accepted basis for definition. It is sometimes difficult even for safety professionals to know when they are talking to a tried and true safety expert or someone just assuming the role. I plan in the coming weeks and months to share highlights of what I believe has become my vision and mission for system safety coming from my 40+ years in the aerospace industry and nearly 20 years as a system safety professional. The reaction to date from my USC students on what I have had to say from has been very gratifying and encourages me to take on this topic in this forum.
