The HRO Theme of the Month for December is Clear Communications. Our actions and words can relay unintended messages depending on the situation, location or the receiver’s perception. However, we generally aim to communicate clearly and for our intentions to be well understood. To do so, we have an opportunity to reflect on the environmental and cultural lenses affecting our messages and to identify the standard language and processes needed to achieve accurate understanding. Continue to Close the Loop A communication breakdown is frequently the leading cause of error—the message gets lost, goes unnoticed, or is misunderstood. At VHA, we can avoid these risks with Clear Communications by closing the loop, which ensures that information is effectively communicated, received and confirmed. Establishing and promoting the communication channels at your facility, VISN or office can ensure their standard use. For example, using the Joint Patient Safety Reporting system to report Adverse Events or Close Calls offers an opportunity to inform all appropriate stakeholders and ensure a more effective response to the risk or error. Additionally, when shift transitions prevent teams from directly communicating, they can employ Visual Management Systems (VMS) to convey the information in a written or visual format. VMS is a critical resource that comes in various forms to support communication across multiple environments. The use of VMS aims to build team cohesion, communicate quality or process improvement tasks, monitor risks and highlight safety-focused topics in a highly visible and at-a-glance manner. VMS may take the form of simple safety signage and color-coded labels for sensitive areas or more complex Continuous Process Improvement or Standard Work boards, for example. Flag if you don’t know Clear Communications removes ambiguity and helps enhance quality and accuracy. You may not know what you don’t know, so it helps to ask. In your role, use the communication channels, tools and processes at your disposal to discover information, ask questions, raise a concern or otherwise communicate what you need. Practice over-communicating When it comes to communication, practice makes perfect. Speaking up feels uncomfortable initially but gets easier the more you do it. The benefit of communicating clearly is that it removes ambiguity and uncertainty and establishes a foundation of accuracy in everything we do. Since we don’t work in a vacuum, communication is critical. Although pitfalls, misunderstandings or even cultural misinterpretations may occur along the way, our ability to continue striving toward inclusive collaboration and Clear Communications will forge significant progress along our VHA Journey to High Reliability.

🕴 New Communication Policy Implementation: 💬 In appreciation of your continued interest and support, we felt it necessary to craft this note detailing some changes to the way we manage communication. As both demands on time and the need for focused deep work escalate, it has become crucial to weave an effective, yet viable, communication policy to ensure that no message, response or request gets unduly delayed or lost in the clutter. To honor the context of the task at hand and concentrate best on high impact projects, Mondays and Tuesdays have been set aside for intense deep work. Mornings from Wednesday through Friday have also been dedicated to tackling large priority tasks and projects. Therefore, you may experience some delay in responses during these periods. To better manage this, we aim to respond to all received emails within a span of 5 business days. The exception being any communication marked URGENT - these will certainly be addressed within 24 hours. If a matter is pressing and there is a need for immediate contact, please dial the office line at 608-235-5046. Do note that it frequently operates in Do Not Disturb mode, so we would appreciate if you could give it three consecutive calls to break through. We deeply appreciate your understanding, cooperation, and patience as we implement this new policy. Despite the changes, we assure you that your concerns and queries remain a high priority to us. We understand any inconveniences this might cause and apologize for them. Bear with us as we navigate these new settings.
