Lateral episiotomy or no episiotomy in vacuum assisted delivery in nulliparous women (EVA): multicentre, open label, randomised controlled trial Lateral Episiotomy in Vacuum Assisted Delivery for First-Time Mothers Objective To compare the effect of lateral episiotomy versus no episiotomy on obstetric anal sphincter injury in first-time mothers requiring vacuum extraction. Study Design A multicentre, open label, randomised controlled trial conducted in eight hospitals in Sweden from 2017 to 2023. Participants 717 first-time mothers with a single live fetus of 34 gestational weeks or more, requiring vacuum extraction. Intervention First-time mothers were randomly assigned to receive either a lateral episiotomy or no episiotomy during vacuum extraction. Main Outcome Measures The primary outcome was obstetric anal sphincter injury diagnosed by combined visual inspection and digital examination. Results The study found that lateral episiotomy significantly reduced the risk of obstetric anal sphincter injury compared to no episiotomy in first-time mothers requiring vacuum extraction. Conclusions Lateral episiotomy can be recommended for first-time mothers requiring vacuum extraction to reduce the risk of obstetric anal sphincter injury. March is National Endometriosis Month, and it's the perfect time to highlight the ground-breaking strides being made in clinical trials for endometriosis. This spring, Andrew Horne, Professor of Gynaecology and Reproductive Sciences at the University of Edinburgh, is moving a clinical trial of DCA in endometriosis cases into its second phase - with hopes the drug could be prescribed as a treatment on the NHS within the next five years. Clinical research is the driving force behind improved diagnostics, treatments, and, ultimately, enhanced quality of life for those affected by this complex condition. #endometriosisawareness #clinicalresearch

[🩸Endometriosis - COEMEC accreditation] The Gynecology and Andrology Department at Cliniques universitaires Saint-Luc has been awarded COEMEC accreditation by the Surgical Review Corporation! 🏆👏 They are now the first French-speaking Belgian center recognized as a Center of Excellence in Multidisciplinary Endometriosis Care. This achievement marks more than 40 years of dedicated efforts in the fight against endometriosis. This hormone-dependent disease is characterized by the abnormal presence of endometrial-like tissue outside the uterine cavity, usually associated with an inflammatory process. Endometriosis mainly causes pain during menstruation and intercourse, and is sometimes associated with infertility. Difficult to diagnose, it can significantly alter the quality of life of patients, impacting them sexually, socially, professionally and psychologically. In Belgium, almost 10% of women are thought to be affected. Multidisciplinary teams are working hard to offer cutting-edge care in line with international standards, improving patients' quality of life. #Endometriosis #WomensHealth #COEMEC #Endometriose

SPECIAL ISSUE ON POST-PARTUM HEMORRHAGE - Heat-stable carbetocin (HSC) and tranexamic acid (TXA) are World Health Organization-recommended medicines that have the potential to address gaps in postpartum hemorrhage (PPH) care in cold chain-challenged settings. Implementation research in varied high-burden settings allows an understanding of the factors that contribute to the successful implementation and integration of recommended interventions into overall PPH care. We are deeply convinced that improvement in clinical care of patients depends on the development of much greater interdisciplinary cooperation, standardization of terminology and diagnostic procedures, and systemization of the scattered medical knowledge. Under this light and according to our experience, in obstetrical care, the definition of a new multidisciplinary field of research to be termed “Obstetric Echodynamics” permits to tailor the management strategies of hypertensive disorders and to screen the patient individual risk according to the hemodynamic phenotype. The Obstetric Echodynamist (gynecologist, cardiologist, radiologist…) through the multidisciplinary cardio-obstetric approach, represents the future clinical support in the maternal-fetal wellbeing management. Thanks to all my colleagues for the conceptualization and realization of this new futuristic project!
