A quick search online reveals that, at least anecdotally, many patients behind their screens are suffering from symptoms resembling angina and anginal equivalents but feel that they are not being given serious attention as patients and thus suffer additionally from a lack of diagnosis (which then equates to a lack of treatment). Clinicians are only just now realizing how prevalent ischemia without any obstructed coronary arteries (INOCA) really is, but clinical practice continues to lag far behind with only a handful of institutions offering the ability to interrogate the microvasculature to get a "complete" picture of patient heart health. Worse still, almost all of our traditional non-invasive frontline tests are notoriously difficult to use consistently for diagnosing CMD at the beginning of a patient's care journey. A unique case published by Kevin Tom and Jeffrey Bzdusek in Cureus Journal of Medical Science of a #heroinoverdose complicated by acute compartment syndrome (#ACS), rhabdomyolysis, and acute renal failure highlights the critical need for vigilance in #emergencymedicine. This 38-year-old patient presented with a CK level of 212,845 U/L, one of the highest recorded. Early intervention with #fasciotomy and haemodialysis was key to recovery. Compartment syndrome, often due to #fractures (75%) or trauma, can also stem from opioid overdoses. Recognising and treating this rare but life-threatening condition is crucial.
