Because of the gender wage gap and because patients still wrongly perceive women as less competent, these studies get published. Thing is, I don’t know that these studies actually convince anyone to give up their biases. If anything, these large #database retrospective studies always have many methodological gaps and flaws where it leads to quibbling over the methods section. For instance this study did not measure or assess case complexity. I wonder about the opportunity cost - what if this time and effort and cost to prove female doctors are valid had been spent on bias *patients* experience? There are so many gaps in #data that patients experience that cause unsafe drugs and unsafe procedures. In an #AI and guideline driven world, these data gaps can block care. For example, insurance uses flawed #bigdata and incomplete evidence to deny thousands of cases at a time. https://lnkd.in/eRJxQY6J When the rules and systems are safe then safety is achieved by “following the rules.” When systems are broken or harmful, then “following the rules” harms. Countering this requires fighting the system. That can be labeled “disruptive.” Women are more rule following and are more quickly penalized if “disruptive” (if question, do not just go along, do not comply, not docile) so I worry about some approaches to trying to validate the competence of female doctors. Can we successfully fight for ourselves to “belong” *and* also fight the system on behalf of patients? When we celebrate women doctors for better following rules, that rewards less questioning of if the rules themselves are wrong. Personally, I found male surgeons more risk tolerant in ways that were more patient centered for my uterine surgery needs. They were willing to do a more complex or risky surgery - like the male surgeon in my 30s who removed 10 fibroids and managed to piece back together my uterus so I keep my uterus. (Many surgeons would have claimed hysterectomy was the only option - I have had many women doctors push me for a hysterectomy claiming that was safest when, in fact, existing and emerging data show many risks to hysterectomy including urinary incontinence and earlier #menopause.) I know my experiences might not be common for female patients but as I am extremely selective in who I choose as my surgeon and I do a ton of research and vetting, I do find those male doctors who listen and engage in shared decision making. Most importantly, I choose surgeons - regardless of gender - who I trust will not let the system bully them. I see that they take their time in patient visits and do full, complete physical exams. Admittedly, I like the so-called “cowboy” or “disruptive” doctor who persists with independent thought with an old fashioned individualized approach to their practice.
