AIN published an article today about an analysis of the July 2021 Challenger accident in Truckee. The analysis was done by the Air Safety Institute (ASI) of AOPA. https://lnkd.in/gQg3pECP https://lnkd.in/gkXiaQBc As contrasted to the NTSB report on this accident, the ASI report stresses the critical importance of energy management during circling approaches while still pointing out the errors that were made by the flight crew. The NTSB report, however, seems primarily focused on blaming the accident on pilot error. One investigator I spoke with even said: ‘Sometimes you can’t fix stupid’. IMHO, engraving ‘Stupid’ on their headstones along with ‘Pilot Error’ doesn’t help anyone. The ASI analysis on the other hand provides important lessons that other pilots can utilize to help avoid a recurrence of the type of poor energy management in the traffic pattern that was seen in this accident. I believe that our existing (non-military) pilot training approach doesn’t stress the harsh reality which is that airplanes really don’t want to fly! They are very, very heavy machines. The only reason they will fly is ‘airspeed’. Piss away your knots and your airplane will drop like a rock. Do it in the traffic pattern and you will most likely be the subject of another NTSB report and you will have the same epitaph.

OTD in 1989 an Air Ontario Fokker F-28 crashed on takeoff from Dryden, ON due to icing, killing 24 out of 69 POB. Safety Topic of the Week: Collision Avoidance Statistic shows that mid-air collisions can occur in all phases of flight and at all altitudes. However, nearly all mid-air collisions occur in daylight and in excellent visual meteorological conditions. A collision is more likely where aircraft are concentrated, especially close to aerodromes, and when one or both aircraft is turning, descending, or climbing.
