📌 New in the Journal of Cardiovascular Electrophysiology "Left ventricular volumes and function in successful and failed His-BundLe Pacing. A comparative prospective study" by Francesco Notaristefano et al Successful HBP was superior to RVP- because of failed HBP- in preserving left ventricular function in patients with normal EF at baseline. Ophthopedia Update:Ocular Neuromyotonia: Clinical features, Diagnosis and Outcomes: Ocular neuromyotonia is an episodic, involuntary sustained contraction of one or more extraocular muscles. It presents with intermittent diplopia often triggered by sustained gaze in the direction of action of the muscle. (1-5) It was first described clinically by Clark in 1966 (6), however, Ricker and Mertens were the first to coin the term ocular neuromyotonia in 1970. (7) Electromyographic studies have demonstrated that the spontaneous discharges from the extraocular muscles are neurogenic in origin, due to hyperactivity of the motor nerve fibers supplied by cranial nerves III, IV and VI. #Ophthalmology #Ophthotwitter #Scicomm

Quick Takes Splanchnic ablation for volume management (SAVM) involves endovascular ablation of the right greater splanchnic nerve with the goal of reducing splanchnic vasoconstriction and thus reducing filling pressures of the heart. SAVM was not shown to improve filling pressures or clinical outcomes in the results of a blinded, randomized, sham-controlled trial of 90 patients with heart failure with preserved ejection fraction (HFpEF). Subgroup analysis suggests patients with HFpEF and preserved cardiac output with exercise, chronotropic competence, preserved pulse pressure from supine to standing, and less severe diastolic dysfunction by echocardiography (E/A <2) may respond to this therapy.

Happy to share our recently published narrative review where we collected al evidence regarding the effect of Spinal Cord Stimulation (SCS) for visceral pain. This was a shared first authorship with Matthanja Waller-Bieze, and published together with Leonardo Kapural, MD, PhD, Sean Li, Kris Ferguson, Ralph de Vries, Michael Schatman, Velja Mijatovic and Jan Willem Kallewaard. We found that SCS could be effective in reducing symptoms and improve quality of life in patients with intractable visceral pain. SCS seemed to have better outcomes when it was applied for endometriosis, compared to other indications. However, since most studies were of low quality we underlined the need for sound, prospective and preferably randomized studies. You can find the full article below; https://lnkd.in/ebP3Yx89

Please kindly check our new publication: https://lnkd.in/gPe2hucr Inherited Primary Arrhythmias Syndromes (IPAS) have been proven to possess arrhythmogenic substrates and/or arrhythmogenic activations characteristic of their respective types, which can be effectively detected through a synergistic combination of substrate-based and activation-based mapping techniques. Moreover, ablation performed at these specific sites has been conclusively shown to diminish VA recurrence and VA burdens, thereby holding promising therapeutic advantages when implemented proactively.

* A really large and multinational study studied patients with DM and POAG (n=44,359) vs DM without POAG (n=4,393,300) retrospectively. * The main objective was to find if those with DM and POAG are at an increased risk of DR, and PDR. * They found precisely this i.e. those who have DM and POAG are at 2.3 times higher risk of DR compared to those without POAG. * So POAG appears to heighten the risk of DR development. This is something we should all be very aware of, and examine POAG patients with coexistent diabetes very closely for DR and PDR development. * Please share this with other ophthalmologists. * Statistical points to understand - odds ratios, risk ratios and hazard ratios are all used and reported in this paper along with their 95% confidence intervals.
