COLLABORATION & EDUCATION How much is TOO much to ask of an interspinous device? 66 yo snowbird who presented to my clinic after failing interspinous spacer placement in her home state. Thankfully, this is something I don't see from my own interventional pain partners here. One of the benefits of our practice at TPC is the close collaboration between myself, in the Spine Surgery, and my interventional pain colleagues. The overlap between interventional pain providers and spine surgeons is more prominent now than ever before. I believe we at TPC are an excellent case study in the benefits of this type of partnership; just as spine surgeons from neurosurgery and orthopedic surgery came together in the last 20 years to learn from one another. In the coming years, we will be struggling to meet the demands of an increasing population in need of the full spectrum of spine care. The sooner we come together and learn from one another; the more informed our surgical decision making is, the better our patients will do, and we can continue to fight against the public perception of "failed back surgery." I have learned so much from my colleagues and I hope that I have made a similar positive impact for their practices. Early on, I spoke on the importance of getting upright x-rays in order to assess instability in our patient's that can be deceiving if making surgical plans are based on supine MRI or CT images. They've been incredibly receptive and implemented this into their protocols. I think this is just one example of the benefits of this inter-specialty collaboration and how it results in better care for our patients. Due to the amount of instability present I opted to revise the area with a navigated, L4-5 OLIF + PSF, along with removal of the interspinous device (patient's request). This is all done single position, lateral in about 2.5 hours. Her 6 week post op images on the right. She's doing fantastic. She's anxious to begin gradually increasing her pickleball activities and planning to get her prepared for kayaking at ~3 months! There are a plethora of misconceptions about orthopaedics. Let's break some stereotypes. One common misconception is that orthopaedic surgeons are focused on surgery, often dismissing non-surgical treatments. In reality, we believe in a holistic approach to patient care. We work collaboratively with physical therapists, pain management specialists, and other healthcare professionals to explore all non-surgical options. Furthermore, orthopaedic surgeons are sometimes labelled the 'carpenters of medicine', focusing solely on mechanical fixes, like screws and plates. This stereotype ignores the complexity of the biological and medical aspects of musculoskeletal health that orthopaedists must understand. Popularized by Sir John Charnley, MB, ChB, MRCS, LRCP, in the 1960s, total hip #arthroplasty with a cemented femoral component was the standard of care for surgical treatment of #osteoarthritis for several decades. However, as cemented implant designs increased, so did failure rates. The use of cemented THA decreased as data demonstrated the success of various cementless THA implants, which remained an outlier until the 1990s to 2000s. In addition to being more efficient compared with cemented THA, James Browne, MD, of orthopedic surgery at the University of Virginia, said one of the reasons why cementless THA became the mainstay in North America was due to the desire to have a biologic interface and improve the longevity of fixation, particularly in young patients. “The use of cement is a great way to anchor a component into the bone, but it is a mechanical method of fixation and, over time, it can loosen and fail,” Browne said. “A biologic method of fixation is more dynamic, [and] it can theoretically last forever.
