How many of your organisation's incident/accident reports consider these contributory factors? How does your 'RCA' capture and plot them? Are they framed around 'non-compliance' and counterfactuals (failed to, should have, could have...) or do your reports and 'investigations' identify, capture and analyse the contextual factors? Having a formal structure that is recognised across the globe helps individuals, teams, and organisations learn at that global level. The goal is learning and improving - not just lessons identified. A key amendment to SOLAS Regulation II-2/3-4.... After finalization by SDC 9 and subsequent approval by MSC 107 of the draft amendments to SOLAS regulation II-2/3-4 to include emergency towing arrangements for ships other than tankers, the Sub-Committee has commenced developing associated guidelines, based on the existing instruments for tankers. The Sub-Committee invited interested Member States and international organizations to submit more information, including data, to facilitate further considerations at the next session of the Sub-Committee (SDC 11) to take place in 2025. The Guidelines are expected to take effect from 1 January 2028, when the above mentioned SOLAS amendments are expected to enter into force. Another interesting read pertaining to Part/EMAR 21 Certification Programme/ Plans, TCB, LOI and SMS (the latter is not in EMAR 21 yet): https://lnkd.in/e9AW3SN7 The fact is: Uncertainty in authority expectations and response time contribute a significant "hazard and risk" to a DO's activities ...one which requires active mitigations .... and this is where the HoAW really earns his/her paycheck. It is not easy when your control is limited.

The FAA has yet to update FAA Order 8100.15 Rev. B Organization Designation Authorization Procedures, to address requirements to eliminate interference with Unit Members performing their authorized functions by the ODA Holder. As a result, they are forced to update the existing notice imposing such requirements with N 8100.19, 9/7/2024. It has been 2 years since the original notice. How long can sub optimal practices percolate before a resultant catastrophic fallout? Whilst the old Heinrich's/ Bird’s triangles have come under some criticism recently (for example by the oil/ gas industry), there remains some pertinent rationale for improving the base of the triangle by providing the highest quality of #ProactivePilotTraining and instilling best practices.
