🩺💼 Reflecting on a Week at the Fracture Management Workshop 💼🩺 Just wrapped up a really engaging fracture management workshop this week, teaching family medicine residents and 4th-year acting interns. Family Medicine is such a vast field, and it's incredible how much there is to learn, making it so appealing. We centered our discussion around the importance of continuously learning new skills. This is so we can practice Family Medicine broadly, as it's meant to be, rather than narrowing our scope and relying on specialists all the time. I guided them through recognizing various fractures and understanding which specific X-rays are needed. But we didn't stop there—we got our hands dirty with some splinting right in our offices. Why, you might wonder? Well, it’s simple—our goal is to minimize unnecessary ER visits, which can really increase the bills for our patients, especially those with high deductibles. Trust me, nobody wants that kind of unexpected financial burden. Moments like this workshop remind me why I chose Family Medicine. It's never boring! Every day brings something new, and there’s always a chance to be a jack-of-all-trades, continually adding to our toolbox to better serve our community. No one compelled me to come to work today. Today was a vivid reminder of the unspoken commitment that comes with wearing the white coat. I began the day with a sore throat, feeling tired, and sneezing, worried yet hopeful that I didn't have COVID-19. A day off to rest was within my grasp; however, there I was, contemplating not my own wellbeing, but rather the impact of my absence on the patients who wait to see me. Testing negative for COVID-19, the flu, and RSV might bring relief to many, but for a doctor, it only amplifies the sense of duty. In a way, the test results acted as a green light not just to continue serving, but to reflect on the demands of this path. Growing up in India, parents often felt that if one achieved good grades, then becoming a doctor was the natural next step. Being a physician, however, is a calling that tests more than our knowledge—it tests our resilience, our ability to shoulder responsibilities far beyond what is easy. It's a choice that goes far deeper than the personal sacrifices we make. It's about the lives that depend on our presence, the trust placed in our hands, and the unspoken promise to always be there, even when the world turns its back. To my fellow doctors, who spend day and night ensuring that no patient is left uncared for, this message is for us. Our journey is more than a profession; it's a lifelong commitment to humanity, a continuous test of our stamina and dedication. 🏋️♂️💪 The Underrated Significance of Sports Medicine Physicians Through various professional interactions, I've recognized a tendency to underappreciate the extensive role sports medicine physicians play. We're often mistakenly pigeonholed as caregivers solely for athletes, but our contribution extends much farther. 🔍 Did you know? The number one indicator of life expectancy isn't solely determined by genetics or age — it's an individual's Cardiovascular Fitness. 🏃♀️💨 Prioritizing our cardiovascular health is essential. Strength goes beyond our physical muscle mass; it encompasses our vitality. By enhancing our cardiovascular fitness, we inadvertently bolster our mental health. This correlation partly explains why physical activity is frequently recommended for managing symptoms of depression. 🩺 In my capacity as a sports medicine physician, I focus on both the prevention and management of injuries across the board, not just for athletes. 🚴 What truly gratifies my professional life is the ability to support athletes in their comeback journeys, as well as guiding lifestyle athletes to re-engage enthusiastically with the activities they love. 🌟 My aim, shared by my peers in sports medicine, is to elevate overall fitness levels, thereby contributing to the enhancement of public health. This effort aims to diminish mortality rates while simultaneously elevating each individual’s quality of life. 🩺 Patient-Centered Approach to Knee Pain: A Case Reflection 🩺 Today, a 50-year-old patient visited my clinic burdened with knee pain and a history of negative healthcare experiences. Carrying a BMI of around 50, fear of judgment had kept her from seeking timely medical advice. She had previously been diagnosed with a meniscal tear and arthritis, given a cortisone injection, and advised to lose weight and pursue physical therapy—yet surgery was never on the table, leaving her feeling undervalued and ignored. In our session, I seized the moment to educate—not just treat. We discussed why surgery isn't always the immediate answer for arthritis, the role of the meniscus, and the significant impact weight loss can have on joint health. Together, we explored practical weight loss strategies and I administered a cortisone injection for immediate relief. We talked through exercise plans tailored to her needs and considered future interventions such as PRP and viscosupplementation. The ability to delve deep into her concerns and tailor our session like this was, unfortunately, a rarity made possible only because two other patients had missed their appointments. After five years in practice, it's evident to me that the overwhelming paperwork burden often restricts our capability to provide the care our patients deserve, resulting in subpar treatment due to sheer lack of time. The patient's question cut deep: "Why haven't other doctors discussed these things with me?" While the easy answer is probably a lack of time. I'm sure many of you can relate. Are administrative duties hindering your ability to provide the best patient care? I believe it’s time to re-evaluate our health care systems and processes, ensuring we have the time necessary to serve our patients thoroughly and empathetically. Embracing the Journey to Serve Athletes and Patients Alike In the realm of medicine, just like in life, choosing a path is often the result of a blend between our passions and where we believe we can make the most significant impact. Throughout my final year of medical school, amid the countless hours of studying, rounds, and patient interactions, an important decision lay before me that would shape my professional journey: Should I take the path of orthopedic surgery for sports medicine or embrace the holistic approach of primary care with a focus on sports medicine. This decision was not made lightly. The appeal of orthopedic surgery, with its intricate procedures was undeniable. Yet, it was the dynamic and deeply personal aspects of exercise and sports medicine within primary care that ultimately captured my heart. Choosing the primary care route, I found a unique opportunity not just in treating, but in truly connecting with patients. It allowed me the privilege to journey alongside them—whether they're taking their first steps towards a more active lifestyle or an athlete striving for peak performance. My role has evolved to not just diagnosing and treating but also educating and empowering individuals to take control of their health through active living. And, when specialized care is needed, I am there to guide them through the maze of medical information to ensure they receive the best care possible. Today, as we stand on the beginning of a new sports season, I am filled with anticipation and gratitude. This season marks my fifth year serving as the team doctor for Georgetown College, an experience that has been both humbling and profoundly rewarding. As the Medical Director, overseeing the health and well-being of hundreds of college athletes, I am reminded daily of why I embarked on this path. I am honored to have the opportunity to influence these young athletes' lives positively. A 60-year-old female with a history of foot drop stumbled and fell, impacting her knee. While no fracture occurred, she developed a growing hematoma, now presenting as an almost 3x3 cm swelling on the medial aspect of her knee. Her injury is 2 weeks old now. Ultrasound findings are consistent with a Morel-Lavallée lesion. We were divided on the risks and benefits of aspiration and are mindful of the concerns regarding recurrence. My concerns center on the risks of infection and recurrence. Currently, i have told her to utilize a compression wrap; however, if symptoms persist, my plan would include aspiration, the administration of D50 for sclerosis, and oral antibiotics.
