It’s no surprise that healthcare workforce challenges remain top priorities for hospitals. The labor shortage crisis could create $77 billion in incremental costs in 2024. Here’s how trends in employment arrangements stack up for hospitals and health systems. 1. Critical non-clinical functions like clinical data analysis are at risk for elimination. 2. Layoffs and workforce reductions damage employee morale and increase the risk of long-term costs. 3. Job changes or process improvements require too much time to materialize. 4. Traditional outsourcing methods eliminate jobs and often replace people with less qualified or skilled people. "Who is your customer?" A hospital CEO asked me this question during an interview for a leadership role in 2009. Without hesitation, I responded, "The physicians." He smiled and said, "Good answer." I got the job. He liked my experience, intellect, decisiveness, insight, and sense of urgency. The job was hell from day one. Turns out, the physicians were not the customers, nor were the patients. This was my first leadership stint working for a for-profit hospital system - and my last. The shareholders were the customers, and the regional CEO was a pawn. I've written about this CEO before. A CEO who turned his back on a physician specialist for years. He provided no support and pressured her to produce high volume. He eventually fired her without warning; I, of course, did cleanup. That terminal day, she had a full clinical schedule with a waiting room of patients. Moments after the patients learned they would not be examined that day, the CEO showed up with a 'measuring tape' mindset. The clinical space would be immediately earmarked for an ancillary service. There was no concern that the hospital system did not have another specialist to cover. The loss was costly, but no one cared. Is this still happening today? Sadly, it is. In healthcare intense competition for resources, political battles, and the resulting organizational cultures serve as major distractions pulling us away from what matters: delivering high quality patient care. Importantly, we need to talk about the impact on patients, clinicians and leaders. There can be a crushing loss of agency that comes with being overpowered by the structure, processes and political interference in health systems. This impacts everyone and challenges our capacity to deliver great care, innovate, or have a voice in one’s own care.
