Safety is the key is the title of Chapter VIII of "A Chemical Engineer's Journey". (https://a.co/d/aqmz2M4) " Regrettably, despite diligent efforts, incidents persist. In this chapter, I aim to transparently recount certain incidents in which I have been involved to provide readers with insight. It's a well-established principle that every incident or accident must be rigorously investigated to determine its root causes. The overarching objective is to institute preventive measures, ensuring the avoidance of their recurrence. This first incident is related to the chlorine gas poisoning of a maintenance worker during the plant startup. Chlorine intoxications tend to occur in facilities that produce liquefied chlorine gas. However, in the case of this plant, as chlorine gas was not handled under pressure, the chances for this incident to occur were minimal. Environmental regulations applicable to chemical processing plants require that storage tanks and transfer of intermediate and final products be installed in containment areas with sufficient capacity to contain at least 1.5 times the volume of the largest tank in the area. This requirement had been considered in the design of the containment area where sodium hypochlorite was produced... Root causes: i) The engineers responsible for designing the drainage systems did not consider the design basis and general plan provided by the process owners. ii) The operating instructions did not cover all safety aspects indicated in the risk analysis for each process section, particularly the prevention of mixing incompatible substances. iii) The mandatory use of escape respirators by plant personnel in case of emergencies had not been established. iv) Redundant chlorine gas analyzers had not been considered.

This course was delivered yesterday to a a group of engineers and production chemists from Africa and Latin America. Although a lot of topics were discussed, two in particular stood out for that course: - The importance of using proper testing technique, particularly the molecular microbiological techniques (MMM) specially in complex and troubled fields. - The many issues that microorganisms can cause, not just MIC. The attendees did their part by sharing their field cases which were resolved in a methodical way. A good safety programme is not enough, it has to be outstanding. An outstanding safety programme prevents the existence of hazards while a good safety programme removes the hazards. I completed a course on safety in the chemical process industries through the AIChE - American Institute of Chemical Engineers academy. This course enlightened me on the equal importance of safety alongside production. Key safety information in the chemical industry is sourced from Manufacturers Safety Data Sheets and Technical Information. I learned about both the Informal Safety Review carried out in labs prior to process upscaling and the Formal Safety Review in large chemical plants. Furthermore, I gained insights into the Vent Sizing Package and its application in reactor design, courtesy of the course. How do you Safely Scale up a Given Chemical Reaction? If you are working in industry and are responsible for safety, how do you safely scale up a given chemical reaction? How do you tackle issues like heat transfer, gas release, exposure control, waste stream issues and more? The core of any process safety study is a correct description of both heat and gas release associated to the reaction. How can these be determined experimentally and how can data be used to ensure a safe scale-up? Each safety study starts with retrieving relevant safety data of your starting materials and reagents, a good starting point is the ECHA website where a tremendous amount of relevant data can be found. When designing or developing a chemical process, it is key to quickly identify “the usual suspects” : certain reagents and types of reactions are notably known for their intrinsic safety issues, identifying them early is crucial to avoid future surprises. The thermal stability of all components involved in the process should be understood, in order to avoid dangerous decomposition reactions. Techniques to study this stability with different degrees of accuracy are available, and it is important to understand how the obtained data can be used in practice to assure a safe process execution. In the past decades, process safety studies have shifted from a focus on the mere chemical reaction inside the reactor to a much broader approach. Incidents with waste streams for instance have gained a lot of attention. Although the consequences of these incidents are most often not as devastating a large scale runaway reactions, they can lead to significant personal or material damage and therefore deserve extra attention. From the more technological side, flow chemistry has been used ever more often in recent years in the pharmaceutical and fine chemical industry to enable reaction pathways which cannot be executed safely in a batch manner. This way, executing highly energetic reactions or the in-situ generation of highly reactive reagents, can become feasible on a large scale. wim dermaut is a guest professor at the University of Antwerp teaching process safety to masters curricula in chemistry and chemical engineering.
