Such an important topic. I got a heavy dose of end-of-life problem-solving with both my mom and father-in-law as they struggled through the last several years of their lives.
I loved Dr. Chris' quote, "Doctors are trained to look after patients’ biology, not their interests." To me, this speaks to the concept of 'lifetime partnership' between patient and provider, which to me is a hallmark of transformative value-based care. The term 'lifetime' is inclusive of end-of-life scenarios and 'partnership' implies thinking about the patients interests (not just their biology, as per what Dr. Chris wrote). Thinking of patient interests opens up all sorts of possibilities for us to reimagine primary care of the future.
Companies like Kaiser Permanente have done a great job of moving the focus of healthcare from downstream/acute care to upstream/preventative care. Now companies like ChenMed and KP are moving even further upstream to transformative care and addressing patients' interests, health-spans, and lifelong aspirations. That's truly encouraging!
