Process Safety - How Design Integrity, Operating Integrity and Maintenance Integrity may have averted disaster on JAL 516 We will have to wait until the investigation of Japan Airlines Flight 516 is complete to fully understand what happened and why. However, some immediate assumptions can be made that relate to great process (systems) safety. Process safety has four components: Design Integrity, Operating Integrity, Maintenance Integrity and Leadership Integrity. All four appear to have worked to prevent disaster in this incident. Design integrity is the intentional design and build to prevent accidents and minimize their impact. Design integrity can significantly contribute to the safe evacuation of crew and passengers. The technical specifications of the A350-900 likely gave passengers enough time to escape. These specifications include smart design factors, including engine firewalls, nitrogen pumps in fuel tanks to prevent immediate burning, and fire-resistant materials on seats and flooring. Maintenance integrity involves the proper and timely adherence to maintenance protocols. The systems that needed to work to avert disaster apparently did. My best guess is that adherence to maintenance routines and the skills of the maintenance crews will be identified as contributor to a successful evacuation. Slides worked. Doors worked. Evacuation systems worked. PA systems worked. Operational integrity is about operating within established limits and according to protocols. From the initial interviews, the flight crews performed admirably. This is no accident. Flight attendants train for disasters. Their competency is regularly evaluated. Even the cadence of their verbal instructions is defined, practiced and evaluated ('heads down, stay down!'). Assessing the impact of leadership integrity will take more time. We'll need to understand what was happening in the cockpit and throughout the JAL organization. What we can assume is that a lot of things went right to avert a disaster. When things go right, you can point to strong process (systems) safety leadership! Some questions to contemplate: 1) How rigorous are the designs of your equipment to prevent bad things from happening? 2) Can you be certain that your critical equipment is maintained according to specification, and that you can prove it? 3) Are the folks that operate your systems trained and competent to manage worst case scenarios? How do you know? 4) Is leadership paying attention to safety critical equipment, maintenance processes and operations? Safety and Risk Management: Emphasize your commitment to safety. Discuss your experience with risk assessment and management, including decision-making in adverse weather conditions, high-stress situations, and during search and rescue operations. Aircraft Familiarity: Thoroughly know the helicopters you'll be operating, including their systems, limitations, and performance characteristics. Be prepared to discuss how you've maintained and ensured the airworthiness of these aircraft in your previous roles. Navigation and Search and Rescue (SAR) Skills: Highlight your navigation skills, especially in challenging terrain and adverse weather conditions. Discuss any experience you have with SAR missions, including your ability to plan and execute search and rescue operations efficiently. Communication and Teamwork: Stress your ability to communicate effectively with crew members, ground personnel, and other agencies during emergency operations. Share examples of successful teamwork and collaboration in high-pressure situations. Superintendents are vital to the maritime industry, they are difficult to find, hard to employ and almost impossible to keep for 3 years +. (It used to be 5!) I have spent 7 years speaking with them every single day. The same 3 things still come up time and time again: 1. Base Salary 2. Package + Allowances (And no I don't mean laptop, a phone and paying for your flight to visit the vessels!) 3. Growth Opportunities Of course, there are other factors like location, vessel types, travelling time etc. But these are the top three across the board. That doesn't mean offering more money than the competition will give you a pick of the litter, but being competitive will increase your available candidate pool. I have seen superintendents moving positions JUST because they were not getting any work-from-home opportunities. 40% of Superintendents said they would not even consider a Job if it did not offer hybrid working. It doesn't mean you need to offer them remote working and pay a 20% pension, but it does mean offering hybrid/flexible working and paying some kind of day rate when travelling will still make you more attractive. You can't guarantee they will be fleet managers when you only have 15 vessels and a fleet manager in place already, but you can add responsibilities over time and set a clear path for progression by having an open discussion about what the options could be in the future. It's not rocket science, it's Shipping. Or don't change anything.......
