Listening to Amy Bush, BS, RN, MJ, CCDS, CCS and Jeff Morris, BSN, RN, CCDS, CCS talk about pediatric CDI at ACDIS. A few takeaways: 1) The audience was asked who was doing pediatric reviews. The largest number of hands I have ever seen to that question went up. It is great to see more traction in the pediatric space. 2) Relationships are key. That has always been a hallmark of best practice programs. Relationships help connect us and drive to a shared goal. 3) Leverage data to track and trend performance. Use it to craft education for your team (providers and CDI/coding) A few challenges to implementing pediatric CDI programs: 1) Admin buy in- Often comes from not understanding the space 2) Training- An amazing stat is 62% of respondents to a poll said that their educational material was homegrown. 3) Staffing- Finding staff with experience is a critical challenge. There is currently no pediatric specific certification. Lots to be done in this space and it is amazing to see the growth in pediatrics. Neonatal care that is not mother inclusive is like working with an eye shut, an ear closed and a hand tied behind your back,. Last Friday it was world prematurity day. A day to raise global awareness of the challenges for women and babies associated with preterm birth, and celebrates the lives of preterm infants. The theme of this year was small actions big impact. Advocating for (premature) babies to be skin-skin with their mothers, early and a lot. It improves outcomes and is good for bonding. I definitely support this call to action. However, I believe it does not reach far enough. In my view it is strongly underestimated is how big the role of mothers actually could/should be, especially in resource constraint settings. Mothers are the hands, eyes and ears of nurses and they are the best advocates for their children. They can handle the caring tasks, resolve issues around sensor placement and oxygen therapy and of course they help to focus on the right patients. When we started the neonatal care unit in Sengerema we included this from the start, ensuring that work remains manageable and promoting bonding. Mothers are eager to learn and to support. The most amazing thing is that very they all support each other. Looking after each others children, providing advice and support. We should not consider mothers just as support to keep these babies warm and breathing. They are the hands we need to support the nurses, the eyes we need to watch the patients and the ears we need to respond to alarms. Luckily the tides are changing. The focus is increasingly on involving mothers early on. But we should not hold back. We need to educate and support them so that they can unburden the nurses and are able to take care of their own children. Not just in the hospital, but also thereafter. “We screen everybody for the need for services, and then we have full-time navigators at every clinic to make sure we can meet with families, we can call families, we can navigate their referrals,” DeSpiegelaere said. “Then we actually partner with agencies that provide free diapers or free strollers, and we physically go get those resources and deliver them to families” (Ricciardi, 2023). Every Child Pediatrics is an amazing organization, and one offering healthcare and medications to any child. Throughout this semester of school, and over the last few weeks, I have been discussing the cost and expense of healthcare for our children. Often kids are underinsured and many parents cannot get the help they need, even with insurance. This organization in Denver, Colorado is truly stepping up and meeting a need for all children, whether they have insurance or not. They provide pediatric health care, despite the ability of parents or patients being able to pay for that care. They will take insurance but are a non-profit that doesn't withhold care, even if a patient or family cannot pay. They will accept insurance and operate on a sliding scale, but they also won't turn patients away if they simply don't have the means. Additionally, they have a nurse navigator who will fight and support each family and patient. The idea of having a nurse navigator who will deliver resources is an incredible idea and a role I would love to support. I know personally that when my daughter was sick with a form of gangrene during her treatments for leukemia and was not improving, our nurse navigator was the one who encouraged me to fight for my daughter and demand a change in her care. She encouraged me to take off my nurse hat and become the Mama Bear I needed to be to fight for my daughter. Nurse navigators see the need and fight to make it happen. The idea of a free clinic for all, and one with a nurse navigator, is just one way we can continue to provide for the kiddos who really need it. Organizations like Every Child Pediatrics should be applauded. As a healthcare organization, how can we model our clinics like this one? As healthcare providers, can we step up and fight with our actions and our finances, helping clinics like this one get started? As I have said some many times before, what are we doing to be the change we want to see? Let's join together and fight to provide care for children, no matter what the cost or the challenge might be.
