Dr. Naiem Nassiri, of The Vascular Care Group’s Connecticut team, successfully completed a first-in-the-world treatment of a chronic left external iliac thrombus in the outpatient OBL setting using the @Vesalio pVasc Thrombectomy system. The pVasc thrombectomy system is developed for the non-surgical removal of thrombi and emboli from peripheral arterial and venous vasculature. Female patient in her mid-40s previously underwent a procedure at a major hospital. She presented to The Vascular Care Group with leg pain and swelling due to clot in her left external iliac artery. She was seen in the office and the decision was made to bring her to the OBL in Darien, CT for treatment. The patient’s clot was approximately 4 weeks old at this point and considered chronic. She was successfully treated with the @Vesalio pVasc Thrombectomy System while awake in the outpatient OBL setting. Quoting: “Veterans Health Administration (VHA) is recognized for its initiative the Surgical Pause, which reduced six-month mortality of patients determined to be “frail” from 25 percent to 8 percent at three VA medical centers. Under the initiative, providers implemented routine frailty screening with the Risk Analysis Index to identify patients at the highest risk of postoperative complications, loss of independence, and mortality. For patients found to be frail, a brief “pause” permits further evaluation. For patients who pursue surgery, multidisciplinary care plans are implemented to reduce frailty-associated risks before surgery, shifting the paradigm and effort to strengthening the frail patient and mitigating potential complications before they happen. Further analysis of more than 50,000 patients found that one-year mortality declined by 4.2 percent. As a result of this success, the Surgical Pause has been replicated across more than 50 medical centers across VA and the private sector.”

The year 2024 saw updates to the Ambulatory Surgical Center (ASC) payment system. New condition code 92 clarifies billing requirements for intensive outpatient program services. Additionally, revised edits were implemented for providers with multiple service locations. Heart Rhythm Society’s survey assessing same-day discharge after EP procedures and implementation in ambulatory surgical centers @hrs_journal 1) It appears that yellow (current practice) and green (yes) bars should be summed relative to red bar (no). 2) The majority of #EPeeps in this survey support AF ablation at ASC. 3) I personally think the opposite of the majority with regard to right vs left sided PVC ablation.
