Our European Society for Paediatric Nephrology (ESPN) research (https://lnkd.in/ep-QirYB) reveals significant variations in pediatric kidney care across Europe. The 2020 survey focuses on care pathways during and outside standard hours, exposing disparities in UTI, SSNS, and AKI management. Results indicate diverse UTI care providers during the day, with a shift to hospital services after hours. In 69% of countries, pediatric nephrologists follow up SNSS cases in hospitals. Community-acquired AKI cases are often admitted to secondary or tertiary care hospitals during working hours. Concerningly, nighttime and WE care pathways show an immediate shift to University Children's Hospitals for AKI, with intensive care paediatricians and nephrologists initiating treatment. This diversity in patient pathways outside regular hours highlights a major weakness in the service chain. Gaps and fragmentation in pediatric health services risk delayed or inadequate referral of European children with kidney diseases to specialists. Addressing these disparities is crucial for ensuring timely and effective care, emphasizing the need for a more cohesive and standardized approach.
