Transitional care nurses can lead to better outcomes for older adult emergency department patients. A study published in Academic Emergency Medicine found that transitional nurse care in the emergency department was associated with decreased readmission of older adults during the index ED visit at two of three hospitals, with sustained reduction for the entire 30-day readmission window at one hospital. Today, Medicare celebrates its 59th birthday. With millions of older adults who depend on Medicare for healthcare, we must seek ways to improve older adult healthcare that lower healthcare costs. The good news is that programs and initiatives that make the emergency department better for older adults can do just that.
