Essential Responsibilities: Evaluates and identifies members needs.Interfaces with Primary Care Physicians, Specialists and various disciplines on the development of case management plans/programs.Monitors and evaluates the effectiveness of the case management plans and modifies as necessary.Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination, discharge planning and obtaining all authorizations/approvals/transfers as needed for outside services for patients/families.Acts as a clinical liaison, per their specialty, with outside agencies such as County CCS, non-plan facilities, outside providers, employers and/or workers compensation carriers and third party administrators.Prepares reports, communicates program changes to appropriate staff and develops protocols in accordance with state regulations.Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and patient is educated and empowered to be responsible for participating in the plan of care.Develops individualized patient/family education plan focused on self-management, delivers patient/family education specific to a disease state.Develops and updates training and educational materials and presents to appropriate staff, members and families.Facilitates patients return to normal daily activities by teaching and making appropriate referrals for outside services/continued care.Consults with internal and external physicians, health care providers, discharge planners, and outside agencies regarding continued care/treatment or hospitalization or referral to support services or placement.May need to facilitate transportation and housing arrangements for patient.Coordinates transmission of clinical and benefit treatment to patie

In the life of a long-term care consultant pharmacist: Monday :) Today I went on-site to a facility we provide services to with my team member and consultant pharmacist, Proshi Anika. It was a rainy and dreary kind of day on my drive to the facility. Literally had freezing rain last night and it rained all morning. The day turned much less dreary and much more fun when I got to the facility! Proshi and I went in and checked in on the nurses, audited medication carts and then met with the DON and did drug destruction. At the end of drug destruction, one of the physicians for the facility came in. This was completely unplanned but we all sat down and discussed psychotropic medications, gradual dose reductions, MDS, and so much more! It was so awesome! With all of us in the office, we were an interdisciplinary team, with the betterment of patient lives in mind and having a discussion on very difficult topics made even harder at times due to current regulations. However, we navigated all the discussions and came up with game plans for documentation, patient care, appropriateness of psychotropic agents, etc. I absolutely love these kinds of interactions because when we all walked away, we were all on the same page, we were all working together with the goal of helping patients in the most effective way possible. After we all walked away, we did several more audits, discussed lots of other things, and then left the facility having had an awesome and impactful day! I can't wait till the next time an interaction like this, with multiple healthcare team members, can occur! I drove away from the facility thinking about this 👇 All of us in healthcare have incredible jobs to do! We are making impacts in people's lives each and everyday! If you are in healthcare as a doctor, PA, NP, nurse, pharmacist, and so many other incredible medical professions, know you can, each day, make a difference! So, 💪 those skills and knowledge you have and make an impact in your patient lives everyday! I wish you all an incredible rest of your Monday!
