When All That Glitters is Gold: A Case of Optical Biopsy in an Esophagogastric Lesion Treated with Endoscopic Submucosal Resection Here a challenging case of dysplasia at GEJ without Barrett esophagus. High resolution imaging is the key for making the difference! A #Fujifilm last generation video gastroscope (EG-760Z by Fujifilm – distal end diameter: 9.9 mm) equipped with the optical magnification system and auxiliary cap for better exposure and characterisation of the area of interest was used. During the EGD we first inspected the gastroesophageal junction (GEJ) under white light (WL) finding a horseshoe slightly elevated area on the right side of the field: after retroflexion, the area was better delimited in the context of a hiatal hernia. Under the WL examination, the area did not show specific irregular features. We consequently applied multi-light technology through different observation modes (Linked Color Imaging – LCI; Blue Light Imaging – BLI). LCI showed a better delimitation of the interested area, clearly defining its borders. Then, we applied BLI mode, to perform a specific evaluation of the pit and vascular pattern. Underwater magnification and optical zoom offered optimal detection and visualization of dysplastic features (slightly villous mucosal pattern and loop-shaped vessels, suggesting the presence of dysplasia). We scheduled an endoscopic submucosal dissection (ESD) of two-thirds of the GEJ's circumference. The pathological report on the resected specimen revealed the presence of HGD without signs of BE and free vertical and horizontal margins, resulting in a curative resection (R0). #highresolution #endoscopy #gastroenterology #fujifilm #esd #diagnosis #thirdspacendoscopy #arnascivicohospital #palermo #sicily
