Our current insurance-pay health care model is SHATTERED! Just like shattered glass, it obliterates your view. There are too many cracks to see reality. An interviewer just asked me why I think ~72k physicians (7%) have left the practice of medicine in the last 2 years. Just speaking for myself and maybe the few hundred other doctors I interact with or know personally... It comes down to compensation... It's labor supply and labor demand. The reason people don't flood into medicine is because there are *many* trade-offs/ risks to undertake both to start the path and to make a career of it (losing out on your 20s, ongoing patient care stress, constant litigation risk, etc) The light at the end of the tunnel has to be much brighter to really encourage and incentivize people to go through such a long pipeline that does not earn the same or similar income, in real dollars, than it did 20 or 30 years ago. There are other issues: insurance, overhead costs, regulation, etc ...but no one is going to seek out the long arduous pipeline unless there is a *remarkably* rewarding career at the end of it. Whose job is it to protect the public from unethical medical device companies? This StatNews piece describes the predatory business practice of Zynex, a company that makes electrical stimulation devices for chronic pain. Note that this is an old technology, and the devices are really cheap to manufacture. You can find them for around $20 online, from an array of manufacturers, and the electrode pads are a few dollars per pack. Zynex’s business model is to get physicians and physical therapists to prescribe their device to patients under the promise that insurance will pay. Then Zynex bills the insurer $2000(!) for the device, plus $220(!!) every single month for replacement batteries and pads. (Devices from other companies are rechargeable so you don’t need batteries.) After insurance companies reject the charges, the company turns around and bills the patients. But not until they’ve racked up many months’ worth of additional charges for the consumables. It's a classic scam, and in a just world, the company’s execs, along with the founder and largest shareholder Thomas Sandgaard, would be sitting in jail cells. Unfortunately, it’s unclear whose job it is to protecting patient from these kinds of companies. Health insurers don’t seem to have any obligation to pursue the companies; they simply deny the charges and move on. Physicians for their part are unwitting accomplices. I would argue that physicians should know better than to allow device reps into their offices, but maybe I’m being unrealistic. It seems to me as if this is an argument for tightened consumer protection laws. What I suspect, though, is that this is a Martin Shkreli moment. In other words, a company taking an exploitative business model that’s widespread in the healthcare industry, and simply turning up the dial. So this case should be a call for reform of the whole industry, not just a stopgap to prevent an extreme case like Zynex. But I predict that the device industry will fight hard against anything that threatens their profits. In the mean time, the company just received FDA clearance for their latest device. Information that the company’s sales reps will undoubtedly exploit to sell the device to even more doctors. #hippocraticcapitalism #ethicalhealthcare #medicaldevices #healthcarefraud
