I can't even begin to explain the negative effects of prolonged exposure to a toxic work environment or a toxic bully. Unfortunately this is all too common in nursing. The litmus test for organizations is how they deal with these people. Do they act accordingly or allow the toxicity to spread?

Nurses, don't eat your young. A common phrase that we here, but rarely see. 'You're lucky you got hired when you did, you'd never get hired based on your finnish'. 'Just give me the phone, you don't know what they are talking about anyway' 'You can't take this patient, its too complicated for you' 'Why don't you already know this? You did go to nursing school, didn't you?' All of these are things I have had said to me since I graduated nursing school in 2018. A little about me, that a lot of my colleagues don't know: I am a registered nurse in both Finland and the US. That means I took the NCLEX exam for my US licence, which has a 30% pass rate for foreign trained nurses like me. I have a master's in healthcare, focusing on global health and crisis management. I love sexual and reproductive health and rights, women's health, and think culture based patient education can solve a lot of our non-compliance issues. I plan on getting my doctorate at some point. I have an idea that combines patient education, cultural competence, and nursing competance that would assist healthcare professionals in addressing nutrition needs of foreign patients who are facing disease and illness. I have worked as a sexual assault and domestic violence advocate, working with 2 difference police departments. I have helped with 3 tech start-ups. While I am not a programmer, I have a strong understanding of the start-up world and what makes them tick. When we constantly hear negative things from co-workers, it can make us forget how competent we are. It is easy to say things to others in frustration, but create a toxic work environment. Kindness matters.

In early December 2023, WHO published their guidelines for non-surgical management of chronic primary low back pain in adults in primary and community care settings. These guidelines are useful for medical doctors, nurses, occupational therapists, physiotherapists, pharmacists, psychologists and community workers. They favour evidence-based and patient-empowering interventions like pain education, physical activity and cognitive behavioural strategies. The Tennessee Board of Nursing has clear grounds to deny, revoke, or suspend a nurse's license, as outlined in state law. These include instances where a nurse has engaged in fraud, committed a crime, demonstrated negligence or incompetence, struggled with alcohol or drug addiction affecting their duties, shown mental incompetence, engaged in unprofessional conduct, or violated regulatory provisions. The scope of these grounds, such as "unprofessional conduct" or being "guilty of a crime," can be broad and subjective. 🏨 Psych holds in the ED While a person is on a psychiatric hold (1799/5150) in the emergency department, the hold does not remove all patient rights. Gone are the days of forcibly catheterizing a psychiatric patient to obtain their urine. ☑️ Fact: the patient on a psychiatric hold is not allowed the leave the facility due to being a danger to self, danger to others, or gravely disabled. They typically must be medically evaluated prior to being able to transport to a psychiatric facility. ⚠️ However, the patient is still free to decline the medical screening examination, medical studies such as lab tests, and refuse to change into hospital-approved garments. In these instances, the primary nurse should educate the patient on why it's important to comply with the medical portion of the examination as well as explore other options to allow the patient to retain their autonomy while staying safe. Perhaps security can wand the patient if they prefer to stay in their own clothes, for example. Perhaps a trade of a television in the room for a blood draw could be arranged. Maybe a coveted ice cream can be borrowed *cough* stolen *cough* from the inpatient floor fridge in exchange for a urine test. Being a nurse means getting to know your patient and coming to understand them as a person. Taking the time to find out WHY they are refusing can often yield a trusting nurse-patient relationship that's beneficial for not only expediting the process but ensuring a more comfortable stay during a terribly-uncomfortable time in the patient's life. Instrumental activities of daily living (IADL) are those activities that allow an individual to live independently in a community. Although not necessary for functional living, the ability to perform IADLs can significantly improve the quality of life. The major domains of IADLs include cooking, cleaning, transportation, laundry, and managing finances. Occupational therapists commonly assess IADLs in the setting of rehab to determine the level of an individual’s need for assistance and cognitive function. IADLs are commonly confused with basic activities of daily living (ADLs). The major domains of ADLs are feeding, dressing, bathing, and walking.
