Confession: I was born a crazy horse girl. 🐴 A genetically recessive trait. Only the few who are - and the parents who bankroll their children’s obsessions - can fully appreciate (and grudgingly finance) this affliction. It’s a rare condition that is equally bougie and humbling. This is a picture of the smallest possible jump. I spent the last year rehabbing a horse. A horse that once was able to jump 5x this height. A horse that injured a leg. Then an eye. Then had cancer. We jumped this little jump over the weekend - and it feels like we medaled in Paris. Why is this relevant for LinkedIn? Because life is measured in increments. In barely measurable forward progress. In both horses and in work. Savor each step. Futility Analysis and a Half Marathon in the Snow Last Saturday I ran my first half marathon in years... in the snow...above 2000M. And if that wasn't enough of a challenge, I had also torn a muscle in my leg 3 weeks before the race. So my final weeks of prep had to fundamentally change from the original plan because I couldn't run at all! Instead, I did a lot of cross training (balance, strength work, just standing on my feet for several hours of the day, and a few hikes of course). And my goal changed--instead of trying to compete (at least in my vintage), I simply wanted to finish...preferably at a run. In drug development, we may start programs where we don't have experience, where we need to be really fast, or where we have reason to believe that "what we have always done" may not be successful. So we have to change strategies. We may also need to build in additional risk mitigation--including implementing futility analyses after a certain fraction of the expected data are accrued. Futility analyses allow sponsors to have an independent group look at the data and, in accordance with a pre-defined statistical analysis plan and criteria, make recommendations about continuing the trial or stopping it because--based on the data available so far--the trial is unlikely to read out positive...that is the new drug does not seem to offer the target benefit. This strategy is not new, but offers an opportunity to mitigate risk to patients (don't expose further to a drug that doesn't work) and to the sponsor (if implemented at scale you can save some money and human resources that can be redeployed elsewhere). Teams may want to continue individual trials in the hopes that their drug will perform, just as runners may want to race simply because. But in both cases, the decision to continue must be aligned with data to achieve the desired outcome. Are you willing to make data driven decisions about futility? Mesotherapy is an amazing technique developed in 1952 by the renowned French physician Michel Pistor, initially intended for managing pain and vascular disorders. Dr. Pistor's work in this field earned him the Legion of Honor, the highest accolade in France. During my studies in aesthetic medicine in Paris, I became acquainted with mesotherapy and even brought along some vials with me. Mesotherapy has evolved into a widely used technique in cosmetic procedures, particularly for fat dissolution in weight loss and cellulite treatment. This method, also known as injection lipolysis, has gained popularity for various non-surgical cosmetic procedures, including fat removal in areas such as thighs, buttocks, hips, legs, arms, and even facial areas like under-eye bags. The effectiveness of mesotherapy lies in the combination of pharmaceutical substances, plant extracts, vitamins, and other ingredients used in the injections. These ingredients vary depending on the indication, with substances like phosphatidylcholine (PC), L-carnitine, aminophylline, hyaluronidase, collagenase, glycolic acid, and many others playing significant roles. Mesotherapy is often referred to as a non-surgical alternative to liposuction, offering contouring and body shaping benefits. However, it's important to manage expectations realistically as multiple sessions may be required, and significant fat removal may not always be possible. Patients are also advised to maintain a healthy lifestyle with proper diet and exercise. Beyond cosmetic applications, mesotherapy has shown promise in treating conditions such as skin aging, alopecia, and even in vitro fertilization (IVF) procedures. Preliminary studies have found hyaluronic acid injections effective for skin rejuvenation, while medications like minoxidil and finasteride are used in alopecia treatment. Moving on to Kybella, it is a revolutionary injectable treatment for reducing fat deposits! Kybella targets fat cells, especially in the lower part of the face, such as the double chin and jaws. However, its benefits also extend to other areas such as knees, elbows, arms, and bra bulge. What makes Kybella effective? It contains deoxycholic acid, a potent ingredient that disrupts fat cell membranes. During the Kybella procedure, multiple small injections are administered into the targeted area. While several sessions may be needed, usually around three or four, the results are often worth it. Don't Let Your Diagnosis Define Your Destiny I want to take a moment to celebrate the triumphs of a true hero, Adekunle Gold, who has inspired us all with his unwavering determination and resilience. As a sickle cell patient himself, he has faced numerous challenges, but he didn't let his diagnosis hold him back from achieving his dreams. Adekunle Gold's journey is a testament to the fact that with the right mindset, anything is possible. Despite the physical limitations and pain that come with sickle cell disease, he has consistently pushed beyond his boundaries to pursue his passion for music( the fact is, it's not going to be easy). So, to all the sickle cell patients out there, I want to tell you that your diagnosis is not a curse. It is not a limitation. It is simply a challenge that you must face head-on and overcome.
