Mitochondrial DNA Deletions in iRBD: Potential Marker but not PD Predictor Rapid eye movement (REM) sleep behavior disorder (RBD), also known as idiopathic REM sleep behavior disorder (iRBD), is a prodromal marker for Parkinson's disease (PD) and Lewy body dementia (LBD). While a proportion of iRBD patients develop PD or LBD, current diagnostic approaches lack reliable biomarkers for risk stratification. This study investigated mitochondrial DNA (mtDNA) deletions in cerebrospinal fluid (CSF) as a potential predictor. Interestingly, researchers observed elevated levels of mtDNA deletions in both iRBD subgroups – those who progressed to PD and those who remained iRBD – compared to controls. However, the current findings appear limited to predicting iRBD itself, and further research is necessary to translate this into PD risk prediction.
