2moIn the undefined generic sense, integrated deterrence always exists and always existed. It is the only real form of deterrence. Nuclear deterrence is not known to have ever changed a decision to act in any crisis known to history. Deterrence is also not always the only factor that matters. Even when the Soviet General Staff (wrongly) believed a NATO exercise had launched a "decapitation attack" on Moscow (something that WAS part of US doctrine at that time), the Soviet head of state did not find it credible we would launch such an attack absent a crisis - so he refused to authorize the general "counterattack" that staff had recommended.
