Medically fragile patients experiencing homelessness all too often receive either a lower level of care (I.e.-congregate shelter, street outreach, etc…) or a higher level of care (SNF, long term care, etc…) than what is needed to support their illness because of the dearth of available resources to fill the gap. We need more programs like these that fill care gaps and to think more broadly about the impact they have on the health and well-being of our communities.

We want all mothers and infants to experience the best healthcare and health outcomes, especially those in at-risk populations. So as part of our ongoing commitment to advancing health equity across the country, Humana funded 3 Kentucky-based community organizations that are dedicated to improving outcomes for Black and African American moms and babies in Louisville, KY. At Axis Community Health, we're committed to ensuring equitable access to quality healthcare for all. That's why we were excited to visit the Sacramento Capitol today, advocating for policies directly impacting our patients and clinic. During our visit, Axis’s Chief Medical Officer, Dr. Dawnell Moody, addressed the critical need to tackle the workforce crisis in primary care in the United States emphasizing the importance of preparing the next generation of primary care providers to serve diverse patient populations. Dr. Moody further discussed how Community Health Centers play a vital role in providing essential and cost-efficient healthcare to millions across the US. She outlined challenges faced by Community Health Centers in California, particularly regarding financial sustainability. Our behavioral health team provided insight into the multi-faceted benefits of integrated behavioral health highlighting our Axis Bridge urgent mental health intervention. Dr. Jennifer Penney, Axis’s Chief of Integration, shared the program’s effectiveness in providing urgent mental health services to individuals of all ages as well as the significant cost savings achieved through Axis Bridge, as well as its role in decreasing hospitalizations and unnecessary emergency department visits within the Tri-Valley. Yet, racial disparities are a powerful force shaping the non-medical drivers of health—like access to safe places to exercise, affordable healthy foods, and accessible health services. EHF CEO Ann Barnes, MD, MPH will join #TribFest24 on Friday in Austin to discuss efforts to battle racial disparities in health care access and outcomes. She'll be part of a panel discussion and live taping of the KFF Health News ‘What the Health?’ podcast with Julie Rovner. At EHF, we're focused on leveling the playing field by promoting health equity, ensuring that everyone, regardless of race or neighborhood, has the opportunity to achieve their best health. EHF is addressing these disparities head-on through initiatives and grant funding that tackle the root causes of poor health — like our work in underserved communities to improve access to healthy food, utilizing community health workers to improve maternal health, and projects that are connecting would-be ER patients with desperately needed nutrition and housing needs. Did you know that the demand for dementia-inclusive home and community care is growing? Despite the existence of innovative models of dementia care and best practice approaches, their implementation is not without challenges. Better education and training focused on foundational knowledge, practical skills, combatting stigma, support for care partners, access to quality care, and equitable treatment of care providers can help achieve a more inclusive, knowledgeable, and supportive system. Camden Coalition has a bit of a cult-like status within complex care models, and the work there is ground-breaking, mission-oriented and thoughtful. Given my admiration for them, the studies showing their lack of demonstrable results point to a simple fact: Care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this [really, any] complex population of high-need patients. Primary care has proven its value. I am more concerned that a lot of the innovative, Specialty-focused models seem to rely on navigation, coordination and peer supports - without learning the lessons that are tried and true about complex populations. The reality is that these populations actually need a place to go when their needs escalate, a primary care provider to help them and real specialist access/integration for their needs. Let's hope that the next wave of these innovative companies learn these old lessons - these populations deserve it. https://lnkd.in/ecsjcbY2
