Medically fragile patients experiencing homelessness all too often receive either a lower level of care (I.e.-congregate shelter, street outreach, etc…) or a higher level of care (SNF, long term care, etc…) than what is needed to support their illness because of the dearth of available resources to fill the gap. We need more programs like these that fill care gaps and to think more broadly about the impact they have on the health and well-being of our communities.

The recently released evaluation of Project Roomkey showed what is possible when there is political will, cross-sector coordination and collaboration, and person-centered approaches to housing and health care.
In this new California Health Care Foundation blog, you can read about two Alameda County Project Roomkey 'sheroes' - Alexis Chettiar and Catherine Hayes. These amazing clinicians leveraged their Roomkey success and learnings to create, in collaboration with Five Keys Schools and Programs and other key partners, transitional and permanent housing for medically frail people. They 'do what it takes' for their patients and are an amazing model that we should be replicating across the state. Read about it below.

This is fantastic to hear about. I wonder what occupational therapists think about this, especially knowing our role in working outside of healthcare settings, promoting health, wellbeing and connection through occupation, for social inclusion? We can look at the excellent work happening in Brazil on the social occupational therapy movement, as an example.
Did you know that occupational therapists are specialists in partnering with you to help you explore occupational opportunities for wellbeing?
Like GP’s ‘prescribe’ medications and, in this example, community-based activities, occupational therapists not only ‘prescribe’ activities with you, but actively work with you to tailor those activities to meet your needs, using evidence-based tools, goal setting measures, opportunities to meet for reviews on your progress through your activity participation, and finally reflect on the journey with you.
Social prescribing is more than just stating an activity will be good and then to just do it and you’ll be ‘better’.
Social prescribing with an occupational therapist will help you guide the process through evidence-based strategies and expertise on understanding how particular activities can foster the right person-environment-occupation fit. In fact, occupational therapists engage in four years of tertiary education (or two-four years through a graduate entry masters or professional doctorate program) specifically on the therapeutic use of ‘occupation’, or the activities that we do.
That’s right, our entire degrees centre on understanding the link between activities and human health, functioning, wellbeing, and quality of life.
Occupational therapists can use social prescribing as an intervention to meaningfully and purposefully enable capabilities and opportunities, as well as resources, within inclusive environments to promote human flourishing through occupation (activities), for social inclusion and deep connections to you as a person (human, occupational, social, and spiritual being), to other people, and to the planet.
GPs, communities, social prescribers, please do not undervalue the role that occupational therapists can play with person-centred and outcomes-based social prescribing.
The tools are at your disposal. You can refer to us through Medicare plans for social prescribing interventions (See Medicare Item 10958 as an example).
I’m happy to chat more about this with potential referrers.

Remember this statistic. It’s the number of people who are served by Health Centers across the United States of America. Over the past 50 + years the movement has become more and more significant and quite frankly, critical.
This movement is like no other. It’s about courage, fortitude, perseverance, and compassion. It’s not a job it’s a way of life. It’s about advocating for all. It’s about being a solution for the ills of the world. It’s about lifting up while strengthening America’s landscape.
With just a little federal funding, every dime is stretched and generating 6 times over. The person wins, the employee wins, the local economy wins, the state wins, and more significantly, the nation wins.
Special thanks to Congress and to all U.S. Presidents over the past 50 years who have allocated funding to this beautiful movement.

📣 To kick off National Health Center Week, August 4-10, the White House issued a proclamation recognizing the key role Community Health Centers play in making sure health care is a right in this country, not a privilege:
"Federally funded community health centers are a lifeline for over 30 million Americans nationwide — providing affordable access to quality medical, dental, and behavioral health services, which save lives. Community health center sites are especially critical for people in Tribal, rural, and underserved communities because they are often the closest and most convenient sources of care. During National Health Center Week, we recognize the importance of the nearly 1,400 federally funded community health centers that provide critical services to Americans. We express our gratitude to their dedicated staff, who tirelessly devote themselves to the well-being of their patients. And we recommit to ensuring that every American has access to good-quality, affordable health care."
