Have you ever wondered how you are described in your medical records by healthcare professionals?
Unfortunately, bias appears in the electronic medical records in many ways, including the free text notes by healthcare professionals. This is one way interpersonal bias is passed along within a healthcare setting- often unconsciously, and with real impacts to patient outcomes and experiences.
One study provides an excellent view into the impact of language used in free text notes on Neonatologist’s perceptions of families in the NICU. The frequency of “negative ratings was significantly greater among American Indian/Alaska Native (AIAN) than other patients.” Parent names, a simple humanizing detail, was less likely to appear in commentaries for AIAN families and the notes were more likely to contain potentially stigmatizing details “such as younger or older parent ages, parental marital status, social work involvement, and custody issues or Child Protective Services (CPS) involvement.”
As healthcare professionals, we hold a lot of power when passing along information about a patient or family to others, either verbally or in documentation. This article supports the need for thoughtful training, consideration, and a change in practice to interrupt the impact of bias in documentation.

These practices will take some unlearning and relearning to integrate into the clinical setting. As healthcare professionals, we can support each other in this unlearning process by seeing the practice as a habit we wish to change. We can catch ourselves, and request our colleagues support us in this practice and replace the language with more humanizing commentary.
